slikt dan Truvada (de officiële merknaam van PrEP -

zal moeten blijven doen. Tenzij – of totdat – we hiv
kunnen genezen. Voor nu gaat het erg goed met hem.’

therapy, he’s not infectious for his partners, and he’s healthy.
I’m sure he’d rather be HIV negative, but for him being
positive just means he takes more than Truvada, it also
means he’ll need therapy lifelong, unless –or until – we nd
a cure. But for now he’s doing really well.’
PrEP isn’t available in The Netherlands yet.
The Aids Fund and STI Aids Netherlands
want the drug to be made available on the
Dutch market this year, and are calling
on Edith Schippers, current Minister of
Health, to make this possible. Louise van
Deth, director of both organizations: ’It’s
unacceptable that PrEP isn’t available in
The Netherlands. Hundreds of new HIV
infections occur on a yearly basis, that could
have been prevented. PrEP offers people
who want to take the responsibility
of protecting themselves and their partners
from HIV the possibility to do just that. It’s
the extra method of prevention we need to
put an end to HIV once and for all.’

Misschien was hij er juist sneller bij doordat hij aan de
PrEP was, en dus vaker werd getest dan gemiddeld?
Anders was hij er misschien pas maanden later achter
gekomen.
You could even argue that because he was on PrEP and was
probably tested more regularly than an average guy, the
infection got caught in a very early stage. If he hadn’t been
on PrEP it might have been a few months later.
‘Of zelfs pas jaren later! Het idee dat het slikken van
PrEP ook motiveert om je regelmatig te laten testen, is
heel belangrijk. Daar komt nog eens bij dat sommige
mensen die zich bij hun huisarts melden omdat ze
PrEP willen, getest werden op hiv, en al seropositief
blijken te zijn. Dus in zekere zin hebben ook zij baat
gehad bij PrEP, zonder ooit een pil geslikt te hebben.’
Heeft PrEP in Amerika, waar het in grote steden goed
verkrijgbaar is, ertoe geleid dat men nu massaal
besmet wordt met andere soa’s?
‘Or years later! The idea that taking PrEP motivates HIV
testing is really important. A number of people who come in
asking for PrEP are bound to be already HIV-positive. So in
a sense, PrEP still provided some benet for them, because
in order to get a prescription for PrEP you need to be tested
for HIV rst. The availability of PrEP inspires people to take
an HIV test, so it provides benets even before the rst dose
is taken.’
Has the use of PrEP in the US caused a signicant increase
in people contracting other STIs?

‘Het aantal mensen dat besmet wordt met gonorroe,
chlamydia en sylis stijgt al sinds halverwege de
jaren ‘90. Dat komt mede door de beschikbaarheid van
medicijnen voor die soa’s, en door ‘serosorting’: dat
is wanneer mensen hun seksuele partners kiezen op
basis van hiv-status. Hoewel ‘serosorting’ misschien
zou kunnen werken om de verspreiding van hiv te
voorkomen, laat het natuurlijk wel toe dat andere
soa’s verspreid worden. PrEP zou er potentieel aan bij
kunnen dragen dat mensen meer andere soa’s gaan
oplopen, maar daar kunnen we nu nog niet veel over
zeggen. Het is vooral belangrijk dat we ons bewust zijn
en blijven van de epidemieën van gonorroe, chlamydia
en sylis die überhaupt in de LHBT-gemeenschap
heersen. Die epidemieën komen niet door PrEP. Het
maakt ook eigenlijk niet uit waar ze door komen, maar
wat we eraan kunnen doen. We kunnen ons laten
testen, en als het blijkt dat we een soa hebben zouden

on the rise since the mid-90s. They’ve been increasing
because of the availability of treatment, and because of
‘serosorting’ practices, where people select their partners or
what they do with their partners based on the perceived HIV
status of their partner. That might be good for preventing
HIV infection, but it obviously allows for other STIs to be
transmitted. PrEP could potentially increase the rate of
transmission of other STIs, but it’s really too early to tell.
I think the main point is that gay men and other sexually
active people should know that there are epidemics
of gonorrhea, chlamydia and syphilis going on in our
communities. Those epidemics are not due to PrEP. The
point is not to gure out what to blame for them, but what to
do about them. We can get ourselves tested, and if we test
positive, we get ourselves treated as quickly as possible, and
let our partners know that they’ve potentially been exposed
to an STI so that they can get tested and treated. The beauty
