A. From the September Avada Webinar, several<br />

points were most salient, including that we,<br />

as practitioners must relate the noise situation<br />

each individual patient experiences back to the<br />

ADE (Activity-Difficulty-Emotion). Practically<br />

speaking, this is why it is so important to have<br />

this information in writing at the point of the ATP<br />

and in Goldmine so that these issues can be revisited<br />

at each ten step service.<br />

The speech science lesson is that noise is<br />

broadband with spikes that can be transient<br />

(sudden and sharp) with amplitude fluctuations<br />

(intensity or its counterpart loudness). Noise<br />

management helps to address the “clangs” in<br />

the noise fluctuations so that the patient can focus<br />

on the conversation and not be distracted<br />

from the noise influences.<br />

Noise management does not affect the signal<br />

to noise ratio, but rather looks at each channel<br />

to preserve as much of the speech signal as<br />

possible. To work with signal to noise ratios,<br />

one must look at directionality. There are two<br />

very specific approaches to noise management,<br />

Modulation Detection and Synchrony Detection,<br />

Synchrony Detection found in instruments<br />

with TriState Noise Management can improve<br />

acceptability of the environments the patient’s<br />

experiences by maintaining more of the speech<br />

signal than Speech-weighted Modulation based<br />

Q. How do I as a practitioner, identify which type of noise<br />

management system would work best for my patient?<br />

noise management. TriState is incorporated<br />

in the K220, K210, Chili SP9, Chili SP7 and<br />

K140. Modulation Detection (M60, Chili SP5,<br />

K130, K90 and K70) has more difficulty finding<br />

the speech signal since both noise and speech<br />

modulation though speech has a higher modulation<br />

rate. You should view the Product Overview<br />

of each product.<br />

Our goal for our patients is to set appropriate<br />

expectations based on their hearing loss as<br />

well as to make speech audible and noise comfortable.<br />

Or to put it another way, to improve acceptability<br />

and reduce annoyance when noise<br />

is present without jeopardizing the speech signal.<br />

Eighty percent or more of the patient’s expectations<br />

come from counseling on appropriate<br />

expectations. This goes hand in hand with<br />

patient counseling as well as the input from the<br />

LACE programs our patient’s experience.<br />

2001. Brenda went<br />

away for a bit to work<br />

on her own business,<br />

but when there was an<br />

opening in our Shelton<br />

location in 2007 as the<br />

Patient Care Coordinator<br />

she came back. It<br />

was an easy decision to<br />

re-hire her. She has a<br />

great work ethic, loves<br />

the patients, gets along<br />

with her co-workers,<br />

and goes the extra mile<br />

to do what ever she is<br />

asked to do. In June<br />

2010, Brenda relocated<br />

to our Bremerton location,<br />

which was much<br />

closer to her home. Now<br />

instead of commuting<br />

every day 150 miles<br />

roundtrip to work in the<br />

Shelton office she commutes 120 miles, all<br />

because, she loves her job and wanted to<br />

work for Avada - What dedication!<br />

Brenda was just married in August to Bob<br />

Byers. A Hawaiian themed wedding made for<br />

a great day in Washington. Their combined<br />

family now totals 6 kids, and 6 grandkids.<br />

Brenda and Bob enjoy attending car shows<br />

and showing off their two beautiful classic<br />

Chevy’s, a ‘56 Chevy and a ‘67 El Camino<br />

(See photo). Brenda often drives one or the<br />

other to work, which always creates new en<br />

counters with people taking a look. Brenda<br />

also keeps busy with gardening, going to<br />

yard sales and second hand stores. Brenda<br />

is very artistic and you would be amazed at<br />

the things she creates with items she finds.<br />

Brenda says: “It is wonderful to have so<br />

many chances to help people. I see each<br />

contact as an opportunity to improve someone’s<br />

lifestyle. It is a great feeling at the end<br />

of the day to know I have been able to assist<br />

someone to experience a better quality of life.<br />
