2. Renal ultrasound (USG) 3 : Proforma It is a valuable initial diagnostic study under any circumstances in which overall renal function is inadequate to perform IVP. USG is the standard method for identifying HN in infancy. The size of the renal pelvis (Antero-posterior diameter) can correlate with the likelihood of obstruction, but this does not diagnose obstruction, nor can it answer whether the HN will improve or worsen. Postnatal USG imaging is usually deferred until day 3 of life, to allow for improvement in the relative oliguria, which could lead to underestimation of the degree of HN. The fetal and neonatal kidney may appear to be hydronephrotic on USG secondary to the sonolucent appearance of the medullary area and pyramids. This appearance tends to resolve after parenchymal maturation, which occurs at 3 months of age. Sequential studies become meaningful, because they define a trend with regard to the HN. Worsening HN usually indicates obstruction, and improved HN suggests the opposite. In neonates and infants, the diagnosis of UPJO has generally been suggested either by routine performance of maternal USG or by the finding of a flank mass. In either setting, renal USG is usually the first radiographic study performed. USG should be able to visualize dilatation of the collecting system. In UPJO, the pelvis is visualized as a large, medial sonolucent area surrounded by smaller, rounded sonolucent structures representing dilated calyces. At times, dilated calyces will be seen connecting to the pelvis via dilated infundibula. Occasionally, a solid- appearing renal cortex can be seen surrounding the sonolucent areas or separating the dilated calyces.
