THE FOLLOWING ISSUES AND/OR DEFICIENCIES IN THE PAPERS ARE NOTED
1. Petitioner states that the proposed conservatee’s assets are held in trust.
Therefore, petitioner is only requesting person only.
2. Petitioner is seeking medical and dementia powers. Capacity Declaration is filed
and supports requests.
3. As dementia powers are sought, Court must appoint counsel for proposed
conservatee. Need report of counsel.
4. Proposed conservatee is not able to complete affidavit of voter registration.
5. Proof of Completion of the Conservatorship Training Class.
6. Need return of citation.
7. Court Investigator report filed 4/15/13
RECOMMENDATION: Need appointment of counsel for proposed conservatee and
need to clear notes. If approved, set hearing for Compliance Review for 12/04/13 for
the filing of the required documents including a Determination of Conservatee’s
Appropriate Level of Care per Pr.C.§2352.5 (form GC-355).
