Said survey shall be conducted every 6 months. Failure to meet<br />
satisfactory level after 1 year shall constitute a ground for an issuance of<br />
warning which shall be remedied within a period of 3 months.<br />
11. Upon written request by the HDMF’s Cash and Administrative Services,<br />
The areas, and locations covered by the janitorial services in the contract<br />
may be changed anytime. The SERVICE PROVIDER obligates itself to<br />
render the services upon written request of the HDNF whenever the<br />
exigencies of the service require.<br />
12. Before the end of each year, the HDMF shall conduct an assessment or<br />
evaluation of the SERVICE PROVIDER’s performance based on the<br />
prescribed set of performance criteria.<br />
13. EVALUATION OF TECHNICAL PROPOSAL<br />
The technical proposal of the service providers will be evaluated using the<br />
parameters prescribed in Annex A.<br />
E. SUPPLIES REQUIRED<br />
1. The SERVICE PROVIDER shall provide on its own account all supplies<br />
necessary, appropriate and incidental to the performance of the job. Refer<br />
to Annex “B” hereof.<br />
2. The following supplies shall be delivered by the SERVICE PROVIDER<br />
free of charge to the HDMF:<br />
a. Provision of raincoats and rain boots for the ground<br />
maintenance;<br />
b. Provision of “wet floor” sign;<br />
c. Provision of mop squeezers<br />
3. The maintenance, repair and replacement of tools and equipment<br />
necessary and incidental to the performance of its obligations shall be for<br />
the account of the SERVICE PROVIDER.
