Said survey shall be conducted every 6 months. Failure to meet
satisfactory level after 1 year shall constitute a ground for an issuance of
warning which shall be remedied within a period of 3 months.
11. Upon written request by the HDMF’s Cash and Administrative Services,
The areas, and locations covered by the janitorial services in the contract
may be changed anytime. The SERVICE PROVIDER obligates itself to
render the services upon written request of the HDNF whenever the
exigencies of the service require.
12. Before the end of each year, the HDMF shall conduct an assessment or
evaluation of the SERVICE PROVIDER’s performance based on the
prescribed set of performance criteria.
13. EVALUATION OF TECHNICAL PROPOSAL
The technical proposal of the service providers will be evaluated using the
parameters prescribed in Annex A.
E. SUPPLIES REQUIRED
1. The SERVICE PROVIDER shall provide on its own account all supplies
necessary, appropriate and incidental to the performance of the job. Refer
to Annex “B” hereof.
2. The following supplies shall be delivered by the SERVICE PROVIDER
free of charge to the HDMF:
a. Provision of raincoats and rain boots for the ground
maintenance;
b. Provision of “wet floor” sign;
c. Provision of mop squeezers
3. The maintenance, repair and replacement of tools and equipment
necessary and incidental to the performance of its obligations shall be for
the account of the SERVICE PROVIDER.
