2. Conformity with technical specifications, as enumerated and specified in
Sections VI and VII of the Bidding Documents (Fill-up and submit Annex A-
Technical Parameters and Annex B-Technical Specifications Forms in
Section VII);
3. Sworn Statement in accordance with Section 25.2 (a)(iv) of the IRR of RA
9184 and using the form prescribed in Section VIII Bidding Forms.

1. Financial Bid Form (using the prescribed Bid Form in Section VIII-Bidding
Forms) and the bill of quantities, price schedules and cost breakdown (using
the forms BF 1, BF 1a, BF 1b and BF 1c in Section VI-Schedule of
Requirements)
NOTE:
The Bidder who will be declared as having submitted the
Lowest Calculated Bid (LCB) is required to submit the
following documents per ITB Clause 29.2
(If possible kindly submit these documents on the day of Bid Opening):
• Tax Clearance per Executive Order 398, series of 2005;
• Latest Income and Business tax returns in the form
specified in the BDS;
• Certificate of PhilGEPS ; and
• BIR Certificate of Registration
