Here, in the area of the spoken word, is where I believe nurses can have a huge impact on defeating stigma in healthcare. No matter how twisted your tongue becomes, person first language is essential because language not only reflects, but also creates reality. The word “wrong” is a good example of creating reality. Wrong is a word I hope you will strike forever from your own vocabulary. Individuals with stigmatized health conditions are constantly asked by complete strangers, “What is wrong with your leg, arm, nose, etc.” Like multiple impressions in advertising, I wonder how many times it takes for an individual with a stigma to hear the word wrong before the person internalizes it? Unfortunately, there are many nonverbal messages that can communicate stigma also; the most obvious among them is staring. Although you can never be sure how people will react to you, as a Quiggle holder myself I can attest to one thing you can be sure of…they ARE going to stare. Sadly, it is not just the public that communicates a message of stigma to the person with health problems; it can also be immediate family, friends, or even healthcare providers themselves. These messages are perhaps even more impactful than those of strangers. Healthcare professionals often fall into the trap of stigmatizing people with health challenges with statements such as “your lung X-ray is here” referring to the person with lung cancer rather than a piece of celluloid. Individuals with incontinence report to the Simon Foundation not only their feelings of shame and embarrassment regarding their incontinence, but how stigmatized they often feel in a healthcare setting.
