Identifying Social Determinants of <strong>Health</strong><br />

> Ask partners <strong>to</strong> think 20–25 years in<strong>to</strong> the future and imagine<br />

how they would like life <strong>to</strong> be different in their community.<br />

> Invite outside speakers who can help inform the partnership<br />

about social determinants of health and how they contribute<br />

<strong>to</strong> health inequities in the community.<br />

> Pictures of people, places, or events that can be used <strong>to</strong> describe or tell a s<strong>to</strong>ry<br />

about the community, such as:<br />

• People talking or greeting one another; people arguing or acting hostile <strong>to</strong><br />

one another.<br />

• Closed schools or businesses, building remodeling, or construction.<br />

• Trees, art or cultural decoration; abandoned cars or litter.<br />

> Documentation (e.g., checklists, inven<strong>to</strong>ries) of observations of people, places,<br />

equipment, maintenance, or aesthetics in the community environment, such as:<br />

• People engaging in physical activities; people driving in cars.<br />

• Absence of grocery s<strong>to</strong>res, supermarkets, and produce markets; presence of<br />

fast food restaurants and convience s<strong>to</strong>res.<br />

• Parks with paved, marked, multi-use trails; playgrounds with broken swings<br />

or rusty equipment.<br />

> Existing evidence: published reviews, gray literature, and views and opinions of<br />

people and organizations affected by the issue.<br />

> Identifcation of health relevance of a policy or project of interest.<br />

> Estimation of the size of health impact of the policy or project of interest.<br />

> Identifcation of key health issues and concerns.<br />

> Take a walking <strong>to</strong>ur of different areas of the community<br />

and ask partners <strong>to</strong> take pictures that represent conditions<br />

or social determinants they would like <strong>to</strong> address in the<br />

community.
