A dermatologic history is similar to other fields <strong>of</strong> medicine and and<br />

includes:<br />

Chief Complaint<br />

History <strong>of</strong> Present Illness: Onset and evolution, Symptom<br />

(itch, pain), Current Treatments<br />

Past Medical History (PMH)<br />

Allergies<br />

Medications<br />

Family History (especially psoriasis, atopic dermatitis, allergic allergic<br />

rhinitis, asthma, and skin cancer, particularly melanoma)<br />

Social History: Occupation, living state (may become<br />

important in the case <strong>of</strong> allergic skin reactions)<br />

Review <strong>of</strong> Systems<br />
