TENDER NOTICE<br />

The Medical Superintendent <strong>ESIC</strong> Model & Super Speciality <strong>Hospital</strong> Asramam,<br />

Kollam invites sealed item rate tenders from registered eligible Civil contractors <strong>of</strong><br />

appropriate class having minimum 3 years similar work experience for <strong>the</strong> following work.<br />

The Medical Superintendent has <strong>the</strong> right to reject any or all <strong>the</strong> tenders without assigning<br />

any reason. The tenders will be opened in <strong>the</strong> presence <strong>of</strong> <strong>the</strong> bidders in <strong>the</strong> chamber <strong>of</strong><br />

Dy. Medical Superintendent as per <strong>the</strong> details given below. The bidder has to be submit a<br />

declaration in Rs. 100/- stamp paper stating to complete <strong>the</strong> work as per tender condition<br />

<strong>and</strong> specification.<br />

TERMS AND CONDITIONS GOVERNING CONTRACT<br />

The tender must enclose a draft/ banker cheque <strong>of</strong> Rs. 20,200/- (Twenty thous<strong>and</strong><br />

two hundred only) as earnest money drawn in <strong>the</strong> favour <strong>of</strong> <strong>ESIC</strong> fund A/c No. <strong>1.</strong> The<br />

EMD deposited with earlier if any will not be adjusted against this tender. Tender without<br />

EMD will not be accepted in any case. EMD to successful tenders will be refunded without<br />

interest after, finalization <strong>of</strong> tender.


Performance Guarantee: - As soon as LOI is issued to <strong>the</strong> successful contractor, <strong>the</strong><br />

contractor will submit a performance guarantee 5% <strong>of</strong> <strong>the</strong> contract amount with in 10days<br />

which will be valid up to 60 days beyond <strong>the</strong> date <strong>of</strong> completion <strong>of</strong> contract. The contractor<br />

shall be responsible for ensuring complaints with labour laws <strong>and</strong> all o<strong>the</strong>r applicable laws<br />

that may be in force from time to time during <strong>the</strong> entire period <strong>of</strong> contract.<br />

Arbitration: - In case <strong>of</strong> any dispute, <strong>the</strong> difference arising out <strong>of</strong> <strong>the</strong> agreement, <strong>the</strong><br />

same shall be refereed to <strong>the</strong> sole arbitration <strong>of</strong> <strong>the</strong> chief Engineer, <strong>ESIC</strong> <strong>and</strong> his decision<br />

shall be binding on both <strong>the</strong> parties.<br />

Delay in completion <strong>the</strong> work: - If <strong>the</strong>re is any delay in completing <strong>the</strong> work<br />

contractor shall be liable to pay liquidated damages in <strong>the</strong> form <strong>of</strong> deduction <strong>of</strong> 2% from<br />

<strong>the</strong> total tendered value per month subject to maximum limit <strong>of</strong> 5%. If <strong>the</strong> work is not<br />

compete with in 3 months <strong>of</strong> extended time, his security deposit will be forfeited <strong>and</strong><br />

tender shall be cancelled. Part work done by <strong>the</strong> agency if any will not be measured <strong>and</strong><br />

paid. There will not be any leniency regarding timely completing <strong>of</strong> <strong>the</strong> work. Work has to<br />

be carried out as per CPWD specification, volume 1 to 7 with correction slips up-to-date.<br />

Draft/ Banker's cheque, which will be related on satisfactory performance <strong>of</strong> <strong>the</strong> contract<br />

(else <strong>the</strong> amount will be confiscated). The tender should include <strong>the</strong> following: -<br />

(1) EMD furnished in accordance with instruction as above (2) Work experience<br />

certificate (3) PAN No. <strong>and</strong> TIN No. (4) A/c No. in <strong>the</strong> Bank with <strong>the</strong> agency (5) List <strong>of</strong> work<br />

completed by <strong>the</strong> agency in <strong>the</strong> last 3 years <strong>and</strong> on going work.<br />

Statement <strong>of</strong> deviation parameter wise from tendered commercial condition, if any.<br />

The agency has to sign an agreement with <strong>the</strong> <strong>ESIC</strong> (<strong>Hospital</strong>) in Rs. 100/- stamp<br />

paper. The details <strong>of</strong> agreement can be seen <strong>and</strong> under st<strong>and</strong> from <strong>the</strong> Engineering Branch<br />

during <strong>of</strong>fice hours. Similar type <strong>of</strong> experience certificate to be produced along with tender.<br />

Copy <strong>of</strong> <strong>the</strong> type <strong>and</strong> class <strong>of</strong> registration certificate enclosed.<br />

List <strong>of</strong> previous experience with name <strong>and</strong> address <strong>of</strong> <strong>the</strong> work executed.<br />

The tenderer should have successfully completed <strong>the</strong> following works in <strong>the</strong><br />

last 3 years:<br />

a. One similar works <strong>of</strong> value not less than rupees 4 lakhs.<br />

b. Two similar work <strong>of</strong> value not less than rupees 2.25 lakhs.


c. Three similar works <strong>of</strong> not less than <strong>1.</strong>80 lakhs.<br />

The Medical Superintendent has <strong>the</strong> right to accept or reject any or all <strong>the</strong><br />

tenders without assigning any reason(s) <strong>the</strong>re<strong>of</strong>. The agency should submit <strong>the</strong><br />

Name, Qualification <strong>and</strong> Experience <strong>of</strong> <strong>the</strong> staff proposed to deploy.<br />

Note:- The work shall be awarded only to <strong>the</strong> contractor based on <strong>the</strong> total<br />

price quoted <strong>and</strong> suitability <strong>of</strong> <strong>the</strong> agency. The work has to be executed as per<br />

<strong>the</strong> conditions attached <strong>and</strong> he has to make agreement with <strong>the</strong> department.<br />

Tender conditions will also be a part <strong>of</strong> agreement.<br />

<strong>1.</strong> Scope <strong>of</strong> work: - The scope <strong>of</strong> work included in schedules consists <strong>of</strong><br />

repair & <strong>maintenance</strong> <strong>of</strong> civil works, water supply, Horticulture, sewage<br />

line <strong>and</strong> measurable civil works for <strong>the</strong> smooth functioning <strong>of</strong> <strong>Hospital</strong>,<br />

Staff Quarters <strong>and</strong> its premises.<br />

2. Materials: - The consumable materials required for <strong>the</strong> repair<br />

work shall be supplied free <strong>of</strong> cost by <strong>the</strong> <strong>Hospital</strong> only for Srl<br />

No.1,2<strong>and</strong> 4-sub (18,19,20,21,22,23,24,25,28,29).<br />

3. Repair works are <strong>of</strong> urgent nature <strong>and</strong> same should be executed<br />

immediately on dem<strong>and</strong>, if fails to execute with in <strong>the</strong> reasonable time,<br />

penalty <strong>of</strong> Rs. 500/- will be deducted for each work from <strong>the</strong> contractor.<br />

4. All tools <strong>and</strong> instruments including ladders <strong>and</strong> safety items required<br />

for satisfactory execution <strong>of</strong> <strong>the</strong> works (cleaning, repair <strong>and</strong><br />

<strong>maintenance</strong>) shall be provided by <strong>the</strong> contractor. Any accident<br />

happening due to <strong>the</strong> non-availability <strong>of</strong> <strong>the</strong> safety items will be <strong>the</strong><br />

responsibility <strong>of</strong> <strong>the</strong> contractor.<br />

5. All <strong>the</strong> demolished materials/ debris to be removed to a distance not<br />

exceeding 200mtrs.<br />

6. All <strong>the</strong> dismantled items are to be returned to store.<br />

7. Contractor will be responsible for maintaining <strong>the</strong> existing grass turfing,<br />

plants <strong>and</strong> all operations till <strong>the</strong> period <strong>of</strong> contract completion.<br />

Shaping, pruning <strong>of</strong> plants <strong>and</strong> turfing to be carried through specialist<br />

agency as <strong>and</strong> when required with out any extra cost till <strong>the</strong> completion<br />

<strong>of</strong> contract period.<br />

8. Skilled labour must be One year experience in hospital is to be<br />

deployed.<br />

9. Contractor shall deploy skilled workers based on <strong>the</strong> nature <strong>of</strong> work.<br />

Work can be seen <strong>and</strong> underst<strong>and</strong> during <strong>of</strong>fice hours from Engineering<br />

wing <strong>of</strong> <strong>the</strong> <strong>Hospital</strong>.


10. Every day contractor or his authorized person should see <strong>the</strong> complaint<br />

lodged by <strong>the</strong> users in <strong>the</strong> register which is kept in <strong>the</strong> administrative<br />

section <strong>of</strong> <strong>the</strong> <strong>Hospital</strong>. After rectifying prescribed complaint <strong>the</strong><br />

contractor or his authorized person should intimate <strong>the</strong> completion to<br />

<strong>the</strong> Engineer-in-charge/care taker<br />

1<strong>1.</strong> Contractor shall responsible for pumping, supply <strong>and</strong> distribution <strong>of</strong><br />

water for smooth functioning <strong>of</strong> hospital <strong>and</strong> staff quarters, Dispensary<br />

around <strong>the</strong> clock.<br />

12. Licensed plumber/I.T.I Certificate holder with minimum one year<br />

experience should be engaged for water supply <strong>and</strong> plumbing work.<br />

Copy <strong>of</strong> <strong>the</strong> ITI or Licence-Certificate must be produced for verification<br />

before execute agreement.<br />

13. Contractor should deploy <strong>the</strong> required staff for maintaining <strong>the</strong> <strong>Hospital</strong><br />

system pertaining to <strong>the</strong> Engineering works <strong>and</strong> payment shall be made<br />

by <strong>the</strong> contractor.<br />

14. Security <strong>of</strong> <strong>the</strong> pumping station is <strong>the</strong> responsibility <strong>of</strong> <strong>the</strong> contractor.<br />

15. Necessary safety precautions to be taken by <strong>the</strong> contractor while<br />

working in <strong>the</strong> multi-storeyed <strong>buildings</strong>.<br />

16. Staff employed by <strong>the</strong> contactor has to obtain permission from<br />

Engineer/ Supervisor in charge <strong>of</strong> <strong>ESIC</strong> to leave <strong>the</strong> centre during his<br />

duty hours. Attendant register has to be maintained by <strong>the</strong> Agency<br />

which will be checked by <strong>of</strong>ficers <strong>of</strong> <strong>ESIC</strong> have kept in <strong>the</strong> reception<br />

counter <strong>of</strong> Model <strong>Hospital</strong> building.<br />

17. Agency has to execute an agreement with <strong>the</strong> hospital on a Rs. 100/-<br />

stamp paper along with a security deposit <strong>of</strong> 5% (Five Percentage) <strong>of</strong> <strong>the</strong><br />

annual contract value.<br />

18. <strong>ESIC</strong> reserves <strong>the</strong> right to terminate <strong>the</strong> contract at any time if <strong>the</strong><br />

tender conditions violated by <strong>the</strong> agency <strong>and</strong> <strong>the</strong> work will be awarded<br />

to <strong>the</strong> 2nd lowest tenderer <strong>of</strong> <strong>the</strong> risk <strong>and</strong> cost <strong>of</strong> <strong>the</strong> present tenderer.


19. Employees <strong>of</strong> ARM (Civil) should be wear uniform with a badge showing<br />

<strong>the</strong> Name <strong>of</strong> employee, for identification which is to be provided by <strong>the</strong><br />

Agency.<br />

20. The agency will be responsible for <strong>the</strong> entire management <strong>of</strong> system.<br />

2<strong>1.</strong> Sewage system includes smooth flow <strong>of</strong> sewage up to S.T.P inlet <strong>and</strong><br />

free from any objectional material <strong>and</strong> if any should be removed<br />

immediately including cleaning <strong>of</strong> sewage pumping stations.<br />

22. Any item <strong>of</strong> work which is not included in <strong>the</strong> schedule has to be<br />

executed by <strong>the</strong> agency as required for which D.S.R /L.M.R rates will<br />

be paid to <strong>the</strong> agency as per <strong>the</strong> rate fixed by Engineering wing.<br />

23. Contractor/his representative should day today monitor for <strong>the</strong> smooth<br />

functioning <strong>of</strong> system.<br />

24. Payment will be made every month on production <strong>of</strong> bills.<br />

25. Item <strong>and</strong> quantity given in item No.4 sub item 1 to 29 is tentative only.<br />

It may very at any extend for which no claim will be entertained.<br />

26. Minimum wages act <strong>and</strong> all o<strong>the</strong>r labour laws are applicable to <strong>the</strong><br />

successful bidder.<br />

27. Suitable good quality tools are supplied by <strong>the</strong> contractor to <strong>the</strong><br />

technicians for all type <strong>of</strong> works.<br />

These conditions will also a part <strong>of</strong> tender agreement.<br />
