TENDER NOTICE
The Medical Superintendent ESIC Model & Super Speciality Hospital Asramam,
Kollam invites sealed item rate tenders from registered eligible Civil contractors of
appropriate class having minimum 3 years similar work experience for the following work.
The Medical Superintendent has the right to reject any or all the tenders without assigning
any reason. The tenders will be opened in the presence of the bidders in the chamber of
Dy. Medical Superintendent as per the details given below. The bidder has to be submit a
declaration in Rs. 100/- stamp paper stating to complete the work as per tender condition
and specification.

TERMS AND CONDITIONS GOVERNING CONTRACT
The tender must enclose a draft/ banker cheque of Rs. 20,200/- (Twenty thousand
two hundred only) as earnest money drawn in the favour of ESIC fund A/c No. 1. The
EMD deposited with earlier if any will not be adjusted against this tender. Tender without
EMD will not be accepted in any case. EMD to successful tenders will be refunded without
interest after, finalization of tender.

Performance Guarantee: - As soon as LOI is issued to the successful contractor, the
contractor will submit a performance guarantee 5% of the contract amount with in 10days
which will be valid up to 60 days beyond the date of completion of contract. The contractor
shall be responsible for ensuring complaints with labour laws and all other applicable laws
that may be in force from time to time during the entire period of contract.
Arbitration: - In case of any dispute, the difference arising out of the agreement, the
same shall be refereed to the sole arbitration of the chief Engineer, ESIC and his decision
shall be binding on both the parties.
Delay in completion the work: - If there is any delay in completing the work
contractor shall be liable to pay liquidated damages in the form of deduction of 2% from
the total tendered value per month subject to maximum limit of 5%. If the work is not
compete with in 3 months of extended time, his security deposit will be forfeited and
tender shall be cancelled. Part work done by the agency if any will not be measured and
paid. There will not be any leniency regarding timely completing of the work. Work has to
be carried out as per CPWD specification, volume 1 to 7 with correction slips up-to-date.
Draft/ Banker's cheque, which will be related on satisfactory performance of the contract
(else the amount will be confiscated). The tender should include the following: -
(1) EMD furnished in accordance with instruction as above (2) Work experience
certificate (3) PAN No. and TIN No. (4) A/c No. in the Bank with the agency (5) List of work
completed by the agency in the last 3 years and on going work.
Statement of deviation parameter wise from tendered commercial condition, if any.
The agency has to sign an agreement with the ESIC (Hospital) in Rs. 100/- stamp
paper. The details of agreement can be seen and under stand from the Engineering Branch
during office hours. Similar type of experience certificate to be produced along with tender.
Copy of the type and class of registration certificate enclosed.
List of previous experience with name and address of the work executed.
The tenderer should have successfully completed the following works in the
last 3 years:
a. One similar works of value not less than rupees 4 lakhs.
b. Two similar work of value not less than rupees 2.25 lakhs.

c. Three similar works of not less than 1.80 lakhs.
The Medical Superintendent has the right to accept or reject any or all the
tenders without assigning any reason(s) thereof. The agency should submit the
Name, Qualification and Experience of the staff proposed to deploy.
Note:- The work shall be awarded only to the contractor based on the total
price quoted and suitability of the agency. The work has to be executed as per
the conditions attached and he has to make agreement with the department.
Tender conditions will also be a part of agreement.
MEDICAL SUPERINTENDENT

CONTRACT CONDITIONS
1. Scope of work: - The scope of work included in schedules consists of
repair & maintenance of civil works, water supply, Horticulture, sewage
line and measurable civil works for the smooth functioning of Hospital,
Staff Quarters and its premises.
2. Materials: - The consumable materials required for the repair
work shall be supplied free of cost by the Hospital only for Srl
No.1,2and 4-sub (18,19,20,21,22,23,24,25,28,29).
3. Repair works are of urgent nature and same should be executed
immediately on demand, if fails to execute with in the reasonable time,
penalty of Rs. 500/- will be deducted for each work from the contractor.
4. All tools and instruments including ladders and safety items required
for satisfactory execution of the works (cleaning, repair and
maintenance) shall be provided by the contractor. Any accident
happening due to the non-availability of the safety items will be the
responsibility of the contractor.
5. All the demolished materials/ debris to be removed to a distance not
exceeding 200mtrs.
6. All the dismantled items are to be returned to store.
7. Contractor will be responsible for maintaining the existing grass turfing,
plants and all operations till the period of contract completion.
Shaping, pruning of plants and turfing to be carried through specialist
agency as and when required with out any extra cost till the completion
of contract period.
8. Skilled labour must be One year experience in hospital is to be
deployed.
9. Contractor shall deploy skilled workers based on the nature of work.
Work can be seen and understand during office hours from Engineering
wing of the Hospital.

10. Every day contractor or his authorized person should see the complaint
lodged by the users in the register which is kept in the administrative
section of the Hospital. After rectifying prescribed complaint the
contractor or his authorized person should intimate the completion to
the Engineer-in-charge/care taker
11. Contractor shall responsible for pumping, supply and distribution of
water for smooth functioning of hospital and staff quarters, Dispensary
around the clock.
12. Licensed plumber/I.T.I Certificate holder with minimum one year
experience should be engaged for water supply and plumbing work.
Copy of the ITI or Licence-Certificate must be produced for verification
before execute agreement.
13. Contractor should deploy the required staff for maintaining the Hospital
system pertaining to the Engineering works and payment shall be made
by the contractor.
14. Security of the pumping station is the responsibility of the contractor.
15. Necessary safety precautions to be taken by the contractor while
working in the multi-storeyed buildings.
16. Staff employed by the contactor has to obtain permission from
Engineer/ Supervisor in charge of ESIC to leave the centre during his
duty hours. Attendant register has to be maintained by the Agency
which will be checked by officers of ESIC have kept in the reception
counter of Model Hospital building.
17. Agency has to execute an agreement with the hospital on a Rs. 100/-
stamp paper along with a security deposit of 5% (Five Percentage) of the
annual contract value.
18. ESIC reserves the right to terminate the contract at any time if the
tender conditions violated by the agency and the work will be awarded
to the 2nd lowest tenderer of the risk and cost of the present tenderer.

19. Employees of ARM (Civil) should be wear uniform with a badge showing
the Name of employee, for identification which is to be provided by the
Agency.
20. The agency will be responsible for the entire management of system.
21. Sewage system includes smooth flow of sewage up to S.T.P inlet and
free from any objectional material and if any should be removed
immediately including cleaning of sewage pumping stations.
22. Any item of work which is not included in the schedule has to be
executed by the agency as required for which D.S.R /L.M.R rates will
be paid to the agency as per the rate fixed by Engineering wing.
23. Contractor/his representative should day today monitor for the smooth
functioning of system.
24. Payment will be made every month on production of bills.
25. Item and quantity given in item No.4 sub item 1 to 29 is tentative only.
It may very at any extend for which no claim will be entertained.
26. Minimum wages act and all other labour laws are applicable to the
successful bidder.
27. Suitable good quality tools are supplied by the contractor to the
technicians for all type of works.
These conditions will also a part of tender agreement.
