was usually followed by a letter to GP 1 which, among o<strong>the</strong>r things, would set out <strong>the</strong> <strong>care</strong><br />

plan.<br />

7.9 Mr Z attended <strong>the</strong> practice ei<strong>the</strong>r on his own or in <strong>the</strong> company of his parents. GP<br />

1 said he had sometimes observed tension between him <strong>and</strong> his mo<strong>the</strong>r <strong>and</strong> fa<strong>the</strong>r though<br />

he stressed that <strong>the</strong> family had a caring relationship. He said Mr Z <strong>and</strong> his fa<strong>the</strong>r<br />

appeared to spend time toge<strong>the</strong>r watching television or shopping <strong>and</strong> that his parents<br />

provided a lot of support to Mr Z.<br />

7.10 The practice records show that Mr Z was issued with six prescriptions for<br />

haloperidol in 2009. In all cases it appears that <strong>the</strong> tablets were dispensed to Mr Z. Mr Z<br />

usually collected his prescription from <strong>the</strong> surgery <strong>and</strong> took it to a pharmacy to have it<br />

dispensed. According to GP 1, he usually did this himself.<br />

7.11 Alternatively, at <strong>the</strong> request of Mr Z or one of his parents, <strong>the</strong> prescription would<br />

be collected by one of <strong>the</strong> two local pharmacies. GP 1 said that sometimes one of Mr Z’s<br />

parents requested his repeat prescription <strong>and</strong> ei<strong>the</strong>r collected it or asked for it to be<br />

collected by one of <strong>the</strong> pharmacies.<br />

Mr Z’s <strong>care</strong> in 2010<br />

7.12 The practice electronic clinical records show that Mr Z attended two consultations<br />

with GP 1 in 2010 <strong>and</strong> that <strong>the</strong> practice raised two prescriptions for his routine tablets for<br />

his schizophrenia.<br />

7.13 Mr Z was issued with a prescription for 60 haloperidol tablets on 3 March 2010.<br />

7.14 His first appointment in 2010 was with GP 1 on 16 March 2010 when he complained<br />

about a minor physical condition. GP 1 recorded observations about his outward<br />

appearance during <strong>the</strong> consultation including that he was ―not anxious‖ <strong>and</strong> ―mentally<br />

appears stable‖ <strong>and</strong> that he was taking his tablets.<br />
