8.19 The letter is copied to lead nurse 1, lead practitioner. It was <strong>the</strong> formal referral to<br />

<strong>the</strong> CMHT.<br />

Comment<br />

We think <strong>the</strong> referral would have been streng<strong>the</strong>ned if locum staff-grade psychiatrist<br />

2 had also discussed Mr Z with o<strong>the</strong>r members of <strong>the</strong> CMHT. Locum staff-grade<br />

psychiatrist 2 followed up <strong>the</strong> referral when he discovered that no action had been<br />

taken. We deal in section 9 with <strong>the</strong> response to <strong>the</strong> referral.<br />

8.20 Locum staff-grade psychiatrist 2 told us about Mr Z’s response to <strong>the</strong> suggestion of<br />

having a <strong>care</strong> coordinator:<br />

―When I suggested <strong>the</strong> idea of him having a CPN he actually agreed to it <strong>and</strong> said<br />

that he preferred that to coming to Manor Gate <strong>and</strong> that he would like to have a<br />

CPN that he would be able to contact from time to time without coming to Manor<br />

Gate because he said he doesn‟t really like coming <strong>the</strong>re.”<br />

8.21 Locum staff-grade psychiatrist 2 next saw Mr Z in his clinic on 7 December 2009.<br />

He appeared to be well <strong>and</strong> had had no fur<strong>the</strong>r contact with <strong>the</strong> police since <strong>the</strong><br />

outpatient appointment in September. Mr Z told locum staff-grade psychiatrist 2 that he<br />

had reduced his haloperidol (from 10mg to 7.5mg) because he had to attend job<br />

interviews. Locum staff-grade psychiatrist 2 wrote in <strong>the</strong> records that Mr Z had no<br />

thoughts of harming himself or o<strong>the</strong>rs.<br />

8.22 Locum staff-grade psychiatrist 2 dictated a letter to <strong>the</strong> GP immediately after <strong>the</strong><br />

appointment but it was not sent until 16 December. Locum staff-grade psychiatrist 2<br />

recorded <strong>the</strong> following plan:<br />
