7.29 GP 1 also told us Mrs Y remained his patient. He was surprised to hear that Mrs Y<br />
had expressed concerns about <strong>the</strong> practice; she had never mentioned <strong>the</strong> matter to him<br />
though she came to <strong>the</strong> surgery regularly. He described her as being guarded with him<br />
about what happened to Mr Z’s fa<strong>the</strong>r <strong>and</strong> <strong>the</strong> whereabouts of Mr Z.<br />
Mr Z’s recollection of events<br />
7.30 Mr Z told us that he probably went for a month without taking his tablets before his<br />
offence. He told us he could not remember going to see his GP about not having a<br />
prescription or whe<strong>the</strong>r he visited <strong>the</strong> practice during this period requesting help.<br />
What <strong>the</strong> pharmacies told us<br />
7.31 We asked <strong>the</strong> two local pharmacies what <strong>the</strong>ir dispensing records showed about<br />
Mr Z. Pharmacy 1 told us that <strong>the</strong> last prescription <strong>the</strong>y had dispensed for Mr Z was on 16<br />
March 2010 for a fungal nail treatment. However, <strong>the</strong> last time <strong>the</strong>y dispensed<br />
haloperidol <strong>and</strong> procyclidine tablets for Mr Z was on 6 January 2010. GP 1’s records show<br />
that he wrote a prescription in January 2010 for 112 tablets of each (a two-month supply).<br />
7.32 Pharmacy 1 confirmed that <strong>the</strong>y had not dispensed ei<strong>the</strong>r of <strong>the</strong> prescriptions<br />
issued by GP 1 in March <strong>and</strong> April 2010. They also confirmed that <strong>the</strong>y had regularly<br />
dispensed medication to Mr Z in 2009 <strong>and</strong> sent us corroborating information.<br />
7.33 Pharmacy 2 told us that <strong>the</strong>y had not dispensed any prescription to Mr Z since 13<br />
May 2008.<br />
Comment<br />
We think GP 1 managed Mr Z appropriately. The practice issued a regular<br />
prescription to Mr Z <strong>and</strong> GP 1 saw him as needed. GP 1 saw him twice in early 2010<br />
<strong>and</strong> recorded observations about his mental state on both occasions.<br />
