2. Process<br />

1) a sufficiently rigorous process has been carried out to produce <strong>the</strong><br />

documents<br />

2) satisfy itself that <strong>the</strong> new Cluster Executive Team is aware of all of <strong>the</strong><br />

key issues arising from <strong>the</strong> documents <strong>and</strong><br />

3) has an appropriate strategy to address any risks or concerns raised.<br />

2.1 PCT Cluster Legacy Documents<br />

All 14 PCT Cluster legacy documents <strong>and</strong> associated PCT reports<br />

were submitted to <strong>the</strong> three <strong>SHA</strong>s within <strong>NHS</strong> North of Engl<strong>and</strong> by <strong>the</strong><br />

5 September deadline, with many of <strong>the</strong>se due to be signed off by <strong>the</strong>ir<br />

relevant Boards during September.<br />

Each <strong>SHA</strong> took its own approach to engagement on <strong>the</strong>se, including:<br />

• Director level review of documentation<br />

• Feedback to PCT Clusters on plans from <strong>SHA</strong> Director Review, with<br />

resubmissions required in some <strong>SHA</strong>s<br />

• <strong>SHA</strong> CEO discussion with PCT Cluster CEOs to cover discussion<br />

on <strong>the</strong> issues to supplement <strong>the</strong> documented processes of<br />

h<strong>and</strong>over.<br />

The process enabled triangulation of data <strong>and</strong> intelligence across <strong>the</strong><br />

Directorates of <strong>the</strong> <strong>SHA</strong>, with all involved having an opportunity to<br />

review both <strong>the</strong> <strong>SHA</strong> <strong>and</strong> PCT information. Much of <strong>the</strong> information<br />

utilised for <strong>the</strong> legacy report had also been discussed with relevant<br />

bodies such as <strong>the</strong> CQC. All parties agreed that <strong>the</strong> CEO level<br />

h<strong>and</strong>over discussions were an important <strong>and</strong> valuable part of <strong>the</strong><br />

process. Both CQC <strong>and</strong> Monitor have had <strong>the</strong> opportunity to review<br />

<strong>and</strong> discuss <strong>the</strong> documentation directly with <strong>the</strong> <strong>SHA</strong>.<br />

Appendix 1 sets out a list of all 14 PCT Clusters within <strong>NHS</strong> North of<br />

Engl<strong>and</strong>. The three <strong>SHA</strong> document libraries are a repository for <strong>the</strong>se<br />

documents <strong>and</strong> <strong>the</strong>ir associated PCT documentation.<br />

2.2 Preparation of <strong>SHA</strong> <strong>H<strong>and</strong>over</strong> <strong>Reports</strong><br />

Each <strong>SHA</strong> <strong>the</strong>n produced <strong>the</strong>ir own <strong>H<strong>and</strong>over</strong> reports based upon <strong>the</strong><br />

information provided by PCTs <strong>and</strong> PCT Clusters. These documents<br />

formed <strong>the</strong> basis for detailed conversations between <strong>the</strong> <strong>NHS</strong> North of<br />

Engl<strong>and</strong> executive team <strong>and</strong> each of <strong>the</strong> outgoing <strong>SHA</strong> executive<br />

teams, supported by signed statements of assurance from <strong>the</strong> Nurse<br />

Director, <strong>the</strong> Medical Director <strong>and</strong> <strong>the</strong> Chief Executive. These were<br />

extremely helpful discussions. The three h<strong>and</strong>over documents were<br />

submitted to <strong>the</strong> final <strong>SHA</strong> Board meetings on <strong>the</strong> following dates:-<br />
