INSTRUCTIONS FOR COMPLETING THE APPLICATION FOR<br />
CHANGE OF OWNERSHIP<br />
1. License or Commission: Check ei<strong>the</strong>r Child Care Learning Center or<br />
Group Day Care Home. A Child Care Learning Center is defined as<br />
providing group care, for pay, without transfer <strong>of</strong> legal custody, for 19 or<br />
more children. Group Day Care is defined as providing group care, for<br />
pay, without transfer <strong>of</strong> legal custody, for 7-18 children.<br />
Commission: A certificate conferring authority to perform various acts or<br />
duties.<br />
**You are required to complete <strong>the</strong> same process whe<strong>the</strong>r you are<br />
seeking a license or commission to operate.<br />
2. <strong>Applicant</strong> Information: The applicant information defines <strong>the</strong> person or<br />
entity that has legal ownership <strong>of</strong> <strong>the</strong> business. (This information will be<br />
<strong>the</strong> same for owner/applicant on page 1 and page 3.)<br />
3. Name <strong>of</strong> Center: Write <strong>the</strong> name <strong>of</strong> <strong>the</strong> center exactly as you want it to<br />
appear on <strong>the</strong> license/commission. Show <strong>the</strong> complete address for where<br />
<strong>the</strong> center will be located including <strong>the</strong> county and zip code. Effective<br />
5/7/09 all centers are required to furnish <strong>the</strong> Department e-mail contact<br />
information so that this agency may contact <strong>the</strong> center and send<br />
information via e-mail. Please be sure to list your e-mail address<br />
accurately in this section. (Rule 290-2-1-.05(f) for GDCH and 591-1-1-<br />
.16(g) for CCLC)<br />
4. Type <strong>of</strong> <strong>Ownership</strong>: Check <strong>the</strong> one that applies to your center.<br />
Remember to attach supporting documentation depending on <strong>the</strong> type <strong>of</strong><br />
ownership.<br />
Individual: Complete this section if one person owns all <strong>the</strong> assets <strong>of</strong> <strong>the</strong><br />
business and is solely liable for all debts <strong>of</strong> <strong>the</strong> business.<br />
Partnership: Complete this section if two or more people own <strong>the</strong><br />
business. A partnership is a voluntary contract between two or more<br />
persons to carry on as co-owners, a business for pr<strong>of</strong>it.
