or <strong>the</strong> next business day after <strong>the</strong> incident.<br />

TRANSPORTATION<br />

A written transportation plan must be included with your operation plan and<br />

be a part <strong>of</strong> your policies and procedures.<br />

Types <strong>of</strong> transportation include:<br />

• Routine transportation<br />

• Field trip transportation<br />

Your written transportation plan includes:<br />

1. Name <strong>of</strong> licensed driver and evidence <strong>of</strong> current driver's license<br />

2. Written transportation agreement with <strong>the</strong> parent<br />

3. List <strong>of</strong> children to be transported<br />

4. Checklist for <strong>the</strong> accounting <strong>of</strong> children<br />

5. Transportation record<br />

6. Vehicle emergency medical information<br />

7. Annual vehicle inspection form<br />

8. Evidence <strong>of</strong> First Aid and CPR training for <strong>the</strong> driver<br />

9. Field trip permission form<br />

Include written procedures for any alternate transportation used, such as<br />

contracted transportation.
