REVISED FORM<br />
Revised PS Form 2181-B, Applicant Drug Screening — Consent and Release<br />
Effective immediately, applicants for <strong>Postal</strong> Service<br />
employment who are being screened for drugs must begin<br />
using the revised edition of PS Form 2181-B, Applicant Drug<br />
Screening – Consent and Release, December 2006. The<br />
updated form contains a new Privacy Act statement and<br />
replaces previous editions, which contained space for the<br />
applicant to list drugs used within the last 30 days. Previous<br />
editions are obsolete and must be disposed of properly.<br />
When to Use PS Form 2181-B<br />
Human Resources personnel coordinate drug screening<br />
urinalysis no sooner than 90 days before anticipated hiring.<br />
Only applicants determined to be drug-free as a result of<br />
the urinalysis are eligible for selection consideration. Use<br />
PS Form 2181-B only for applicant drug screening. <strong>Postal</strong><br />
Service employees who apply for promotion, reassignment,<br />
or change to a lower level are not considered applicants for<br />
drug screening purposes, except for positions in the U.S.<br />
<strong>Postal</strong> Inspection Service.<br />

Ordering PS Form 2181-B<br />
Federal Employees Dental and Vision Insurance Program<br />
Coverage under the Federal Employees Dental and<br />
Vision Insurance Program (FEDVIP) was effective on<br />
December 31, 2006.<br />
Plan Updates<br />
The Office of Personnel Management (OPM) has issued<br />
the following FEDVIP updates:<br />
MetLife Dental<br />
MetLife’s new, streamlined process for coordinating<br />
benefits became effective on <strong>February</strong> 5, <strong>2007</strong>. MetLife<br />
coordinates benefits according to the FEDVIP law, which<br />
requires the Federal Employee Health Benefits (FEHB)<br />
plan to pay first. MetLife’s new process minimizes the<br />
administrative burden for enrollees. When an enrollee<br />
receives services from a MetLife provider, the following<br />
steps will occur:<br />
1. The MetLife dental provider files a claim for services<br />
with MetLife.<br />
Health Units and Personnel Offices may order PS Form<br />
2181-B from the Material Distribution Center using touch<br />
tone order entry (TTOE): Call 800-273-<strong>15</strong>09.<br />
Note: You must be registered to use TTOE. To register, call<br />
800-332-0317, option 1, extension 2925, and follow the<br />
prompts to leave a message. (Wait 48 hours after registering<br />
before placing your first order.)<br />

2. MetLife reviews the claim for FEHB information and<br />
checks to see if the FEHB plan pays dental benefits.<br />
a. If the FEHB plan does not pay dental benefits,<br />
MetLife processes the claim and pays the benefits<br />
due to the enrollee.<br />
b. If the FEHB plan pays dental benefits, MetLife<br />
sends the claim to the FEHB provider for payment<br />
and then pays the claim based on the plan<br />
allowance.<br />
3. MetLife sends an explanation of benefits to the<br />
enrollee and the dental provider for information purposes.<br />
The MetLife enrollee does not have to contact<br />
his or her FEHB plan to pay the claim or obtain an<br />
FEHB explanation of benefits.<br />
MetLife enrollees are strongly encouraged to provide<br />
their FEHB information at the point of service and to review<br />
and update their FEHB information at www.BENE-<br />
FEDS.com if it was not previously provided or has changed
