Date of Disposition DEL ECEAP-Heaci Start Data Share Agreement ATTACHMENT 2 CERTIFICATION OF DATA DISPOSITION All copies of any data sets related to DEL Contract # 12-1180 have been wiped from data storage systems. All materials and non-wiped computer media containing any data sets related to DEL Contract # 12-1180 have been destroyed. All copies of any data sets related to DEL Contract # 12-1180 that have not been disposed of in a manner described above, have been returned to the Contractor's Contract Manager listed in this Contract. DEL FrPAP-H ad Start Data Share Agreement ATTACHN1ENT 3 DESCRIPTION OF DATA Washington State Department of Early Learning 1. DEL will provide CONTRACTOR data once a year, no later than March 30th: 2. DEL shall provide the Contractor the following eligible client data elements sorted by zip code within the birth date range: a. Head of Household's first and last name; b. Head of Household's mailing address; C. Primary language spoken in household; d. Type of assistance program received; e. Birth date of eligible child/children; and f. Total number of households per zip code. 3. The DEL Data Custodians are: a. Joyce Kilmer, or her successor, ECEAP Administrator. b. Jennifer Jennings-Shaffer, or her successor, Head Start State Collaboration Office Project Manager
