Once a copy of page 5, 1.6 Declaration of this Toad Dissection Application has been faxed to
the Education Queensland Animal Ethics Officer, your school has approval from the
Queensland Schools Animal Ethics Committee (QSAEC) to undertake:
Toad Dissection with students from Years 8 to 10.
As part of the QSAEC Application Approval requirements, YOUR SCHOOL MUST REPORT
EACH YEAR to the QSAEC once the approved animal use activity has been completed.
Simply fill in this form on completion of the approved Toad Dissection activity and email, fax or
