such incident. The Root Cause Analysis (RCA) is a<br />
methodology commonly used to investigate critical incidents.<br />
This paper analyzes ten methodologies of RCA, five of which<br />
are specific from healthcare and five are from general<br />
applications. The methodologies were analyzed and grouped in<br />
five stages. The ten methodologies were compared for each of<br />
the five stages, aiming to identify the commonalities and<br />
differences. The findings are discussed, trying to highlight<br />
important points that can impact the effectiveness of RCA.<br />

Abstract: The study of interaction of radiation with matter is a<br />
field of great interest in the medical physics. The computational<br />
simulations tools as Monte Carlo have been used in this field.<br />
The Geant4 code is a toolkit that have incorporate several<br />
physical processes and particles that allow its application in<br />
medical physics, in particular, in the area of radiotherapy. In<br />
this work we performed simulations of monoenergetic electrons<br />
beam in the energy range used in radiotherapy, and compared<br />
the results of range of electrons in the water with the results<br />
published by the National Institute of Standards and<br />
Technology (NIST). The simulated results of range were very<br />
close of published results showing that Geant4 is appropriated<br />
to electrons simulations with energy in the range of<br />
radiotherapy.<br />

Abstract: Attempting to guarantee the quality of infusion<br />
pumps, a study was performed at a Brazilian public hospital<br />
applying the Healthcare Failure Mode and Effect Analysis<br />
(HFMEA). This technique can be applied for usage by the<br />
clinical engineering, to identify the failure modes of<br />
electromedical devices (EMD) and to assess the involved risks.<br />
Through this study, problems that interfere with the<br />
performance of infusion pumps were identified and<br />
subsequently some actions were proposed to help reduce the<br />
risks of these potential problems. These actions certify the safe<br />
functioning of this device within the standards of safety and<br />
quality.<br />

Abstract: With rising costs and revenues, somehow stagnant<br />
due to heavy dependence on Health Plans, the combination of<br />
technical quality with the most rigorous financial controls<br />
became necessary standard of Health Establishments seeking to<br />
remain on the market. The lack of a scientific method to<br />
quantify the financial risk of investing in a particular health<br />
sector in Brazil, or in a aprticular region of the country, makes<br />
the discount rates (interest) askek by financing agents to be<br />
defined in terms of improperly generalized indices, which do<br />
not reflect the actual risk of investment. The purpose of this<br />
study was to use the CAPM (Capital Asset Pricing Model) to<br />
determine the financial risk of investing in a particular health<br />
sector, and this tool showed promising to set the actual viability<br />
of the investment and time to return it.<br />

Abstract: In this work a classification of medical equipment<br />
based on corrective maintenance data (number of calls, time<br />
spent and cost) was developed. Classes were established by the<br />
Always Better Control analysis. Paraconsistent annotated logic<br />
was used to identify inconsistencies and indeterminacies in the<br />
classification. As an example, only 42.5% of the syringe<br />
infusion pumps were classified as A (the best classification),<br />
but with a high number of indeterminacies, which was taken as<br />
an alert for further examination. A drill down procedure<br />
showed that only one factor (maintenance time) affected<br />
classification of the pumps. This approach may be helpful for<br />
planning equipment acquisition and replacement, as well as<br />
maintenance management.<br />

Abstract: Urodynamics is the gold standard procedure for<br />
diagnosing bladder outlet obstruction although it is invasive,<br />
expensive, time consuming and is not free of complications. In<br />
a previous work, we reported an alternative method developed<br />
for minimally invasive measurement of the vesical pressure.<br />
The method consists of a device named urethral connector and<br />
a portable system for recording the pressure. In the current<br />
work we aimed at reporting some clinical tests of the method.<br />
Its results were compared with the results obtained by using<br />
conventional urodynamics<br />
