Facilities Fund. Rolka Lou& Saltzer Associates administers this Fund p mut to the direction
of the Arkansas Public Service Commission effective January I, 2004.
The Arkansas General Assembly enacted the Extension of Facilities Fund pursuant to Act
1 771 of 2001, which is codified in Ark. Code Ann. $23-1"1404(e)(S). The Fund provides grants
to extend wireline telscommunications facilities to persons who currently do not have access to
wireline telephone service. The Act requires that notice be provided to members of the General
Assembly whenever grants are made in their districts.
We are in receipt of the following grarlt extension request that was submitted by
CenturyTel of Arkansas, Inc., the designated wireline telephone company, on behalf of your
