8 Chief Psychiatrist’s annual report 2008–09 Figure 6: Duration of restraint episodes, 2003–04 to 2008–09 As shown in Figure 6, most restraint episodes were under four hours in duration (80 per cent of episodes in 2008–09, down from 91 per cent in the previous year). Five per cent of episodes lasted between four and 12 hours (down from seven per cent in 2007–08). The proportion of episodes of restraint exceeding 12 hours in duration increased from two per cent in 2007–08 to 15 per cent in 2008–09. The median duration of restraint was 220 minutes in adult mental health services, 165 minutes in aged persons mental health services and 90 minutes in child and adolescent mental health services. 2.3.2 Persons restrained There were 180 persons restrained in 2008–09, constituting a three per cent increase compared to 2007–08, when 174 persons were restrained. Approximately one per cent of patients hospitalised during the reporting period were mechanically restrained. As Figure 7 shows, almost half of those restrained (45 per cent) would have experienced a single episode of restraint in the course of their admission. Fifty-five per cent of clients who were restrained experienced multiple episodes of mechanical restraint in the course of their admission. A small group of patients with highly problematic behaviours accounts for a high proportion of all restraint episodes. For example, in 2008–09 one single client accounted for 13 per cent of all restraint episodes and for 56 per cent of the total duration of restraint provided across the state. Reviews of the data undertaken by the Office of the Chief Psychiatrist identify outliers of this kind for clinical discussion with the relevant mental health service. This ensures that appropriate reviews occur, including seeking a second opinion, so that treatment and care are provided in the least restrictive manner possible, in accordance with the Act and the Victorian Charter of Human Rights and Responsibilities Act 2006.
