Why Choose Health <strong>Plan</strong> <strong>of</strong> San Mateo (HPSM)?<br />

Choose HPSM and get the health care you need from the community<br />

doctors, clinics, and hospitals you know! HPSM has operated locally in<br />

San Mateo County since 1987. We work with most <strong>of</strong> the local doctors and<br />

hospitals in the area and chain and independent pharmacies. Our doctors<br />

speak several languages, understand your culture, and are close to your<br />

home or work.<br />

If you choose HPSM, the Community Provider <strong>Plan</strong> in San Mateo County,<br />

your monthly premium will be less than if you choose another <strong>Healthy</strong><br />

<strong>Families</strong> <strong>Plan</strong> in the county.<br />

<strong>Healthy</strong> <strong>Families</strong> monthly premiums are based on income and family size.<br />

The range is $4 to $21 per child per month to a maximum <strong>of</strong> $63 per month<br />

for all children in a family.<br />

How the <strong>Plan</strong> Works<br />

Every HPSM <strong>Healthy</strong> <strong>Families</strong> member chooses a Primary Care Physician<br />

(PCP). Your child’s PCP takes care <strong>of</strong> most <strong>of</strong> your child’s health care<br />

needs. If there is an emergency, call your child’s PCP anytime, including<br />

nights and weekends.<br />

We can help you find the right PCP. You can change your child’s PCP as<br />

<strong>of</strong>ten as once a month. If needed, your child’s PCP will refer you to one<br />

<strong>of</strong> HPSM’s specialists. Members can get family planning and prenatal<br />

care without a referral. If your child needs medicine, the doctor will write<br />

a prescription. Once your child enrolls you will receive a packet with<br />

membership information including a Member ID card. You should carry<br />

this card with you to all your child’s doctor appointments and to pharmacies<br />

to get your child’s prescriptions.<br />

How to Choose<br />

To become an HPSM <strong>Healthy</strong> <strong>Families</strong> member, write HPSM in the space<br />

provided on the form.<br />
