How the <strong>Plan</strong> Works<br />

• When you enroll, choose a doctor that is right for your child. We<br />

can help you find a doctor, and you can change doctors for any<br />

reason.<br />

• Your doctor will help with all <strong>of</strong> your child’s health care needs and<br />

refer you to a specialist or hospital when needed.<br />

• We will mail you a member I.D. card. Keep this card with you and<br />

show it when your child goes to the doctor, hospital or pharmacy.<br />

• For after-hours care, call our 24-hour telephone helpline (for<br />

emergencies, call 911, or go to the nearest hospital emergency<br />

room).<br />

How to Choose<br />

Write CalOptima and your choice <strong>of</strong> doctor in the space provided on the<br />
