疝 氣 是 一 種 常 見 於 任 何 年 齡 之 疾 病 , 當 體 腔 內<br />
的 器 官 通 過 身 體 腹 壁 的 缺 口 移 動 到 外 層 時 ,<br />
便 稱 為 疝 氣 。 疝 氣 常 出 現 於 患 者 站 立 或 咳 嗽 時 ,<br />
在 其 皮 下 突 出 一 團 軟 組 織 , 普 遍 在 患 者 的 腹 股<br />
溝 、 肚 臍 週 邊 或 以 往 手 術 切 口 的 位 置 。 當 患 者 咳<br />
嗽 或 抬 舉 重 物 時 , 疝 氣 鼓 出 的 組 織 會 特 別 明 顯 。 平<br />
臥 或 稍 作 按 摩 都 可 令 大 多 數 突 出 的 部 份 在 短 瞬 間 復<br />
位 。 倘 若 疝 氣 不 能 復 位 , 患 者 便 要 進 行 緊 急 外 科 修<br />
補 手 術 令 其 復 位 。<br />
疝 氣 所 引 起 之 問 題<br />
所 有 疝 氣 均 需 透 過 外 科 修 補 手 術 始 得 以 根 治 , 而 且<br />
成 功 率 相 當 高 。 但 若 不 加 理 會 , 疝 氣 不 單 令 患 者 感 到<br />
不 適 、 造 成 煩 擾 , 當 疝 氣 變 得 越 來 越 大 、 不 能 復 位 ,<br />
通 常 潛 在 的 危 險 是 會 把 小 腸 卡 住 , 造 成 小 腸 梗 阻 ,<br />
嚴 重 者 , 更 會 引 致 小 腸 壞 死 。 患 者 若 在 不 能 復 位<br />
的 疝 氣 位 置 上 過 度 施 加 壓 力 , 更 會 導 致 小 腸 穿 孔 。<br />
此 外 , 腹 股 溝 疝 氣 突 出 之 組 織 往 往 會 垂 至 陰 囊 部 份 ,<br />
造 成 明 顯 的 變 形 。<br />

為 使 所 有 由 疝 氣 引 致 的 問 題 得 以 解 決 , 採 取 外 科 疝 氣<br />
修 補 手 術 是 必 須 的 。 一 般 來 說 , 治 療 疝 氣 可 採 用 兩 種<br />
手 術 方 式 : 傳 統 開 腹 疝 氣 修 補 手 術 及 腹 腔 鏡 疝 氣<br />
修 補 手 術 。 採 用 傳 統 開 腹 手 術 方 式 , 外 科 醫 生 會 在<br />
患 者 的 疝 氣 部 位 皮 膚 上 作 一 個 五 至 十 公 分 的 切 口<br />
進 行 修 補 。 而 腹 腔 鏡 疝 氣 修 補 手 術 , 在 過 去 十 年 間<br />
已 發 展 成 熟 , 外 科 醫 生 只 須 透 過 在 肚 臍 上 的 一 個<br />
一 公 分 切 口 及 兩 個 於 下 腹 部 的 半 公 分 穿 刺 孔 , 便 能<br />
完 成 疝 氣 修 補 。<br />
疝 氣 手 術 ( 內 鏡 放 置 位 置 )<br />
腹 腔 鏡 疝 氣 修 補 術 是 怎 樣 進 行 ?<br />
進 行 腹 腔 鏡 疝 氣 修 補 手 術 時 , 病 人 需 要 接 受 全 身<br />
麻 醉 。 外 科 醫 生 首 先 會 在 病 人 的 肚 臍 下 半 邊 緣 作<br />
一 個 一 公 分 的 切 口 , 用 來 放 置 已 接 駁 攝 錄 機 及 光 源<br />
之 腹 腔 鏡 。 隨 後 , 外 科 醫 生 會 在 病 人 的 腹 壁 與 腹 膜<br />
之 間 創 造 一 個 空 間 , 透 過 攝 錄 機 投 射 在 螢 光 幕 上 的<br />
情 況 , 清 楚 找 出 腹 壁 內 的 缺 損 及 疝 氣 的 位 置 , 通 過<br />
兩 個 於 下 腹 的 半 公 分 穿 刺 孔 將 兩 枝 細 長 的 儀 器 放 入<br />
腹 壁 內 , 在 疝 氣 缺 損 的 位 置 上 進 行 修 補 , 並 放 入<br />
一 塊 補 片 覆 蓋 疝 氣 缺 損 的 位 置 。 而 補 片 會 在 病 人<br />
復 原 期 間 強 化 腹 壁 內 的 缺 損 , 使 疝 氣 能 得 到 完 全 的<br />
修 補 效 果 。

Laparoscopic hernia repair is a reliable way to<br />
fix the hernia using minimally invasive principles.<br />
With minimum trauma associated with the method, the<br />
patient usually feels minimal pain and can be up and<br />
about right after the surgery. The patient can usually<br />
get back to normal daily activities the day after surgery<br />
and return to work or sports a few days after surgery.<br />
The tiny scars resulted from the small incisions in<br />
laparoscopic hernia repair is much better looking than<br />
the longer surgical scars of the open surgical method.<br />

shown that the recurrence rate following laparoscopic<br />
repair can be kept at a very low level when compared<br />

Hernia is a common condition that occurs in all<br />
ages and is amenable to surgical treatment with<br />
a high success rate. Hernia presents as a bulge<br />
underneath the skin which frequently appears on<br />
standing up and when one coughs. Hernias commonly<br />
appear in the inguinal region, at around the navel and<br />
at sites of previous surgical incisions. All hernias are<br />
formed because of an underlying deficiency in the<br />
abdominal muscle wall. The bulge of the hernia can be<br />
made prominent when one strains the abdomen during<br />
cough or weight lifting. Most hernias can either be<br />
reduced spontaneously when lying in bed or with<br />
a gentle massage on the hernia. If the hernia cannot be<br />
reduced, it requires urgent surgery for reduction.<br />
Problems Associated with Hernia<br />
All hernias will not be resolved without surgical<br />
treatment. If left untreated, the hernia may get<br />
bigger in time and become irreducible. There is<br />
always a danger of trapping the intestine in the<br />
hernia leading to blockage of the gut, and in severe<br />
cases, endangering the viability of the bowel.<br />
Hernias usually get bigger and cause a lot of<br />
discomfort and inconvenience to patients. In inguinal<br />
hernias, the groin bulge can extend downwards to the<br />
scrotum and cause significant disfigurement. Undue<br />
external pressure on the hernia may cause injury<br />
or even perforation of the bowel inside the hernia,<br />
especially if the hernia cannot be reduced. In order to<br />
solve all problems associated with hernias, surgical<br />
repair is necessary.<br />

Ways to Fix a Hernia<br />
There are two ways to fix a hernia in general: the open<br />
surgical method or the minimally invasive approach<br />
with the use of laparoscopy. The open surgical method<br />
will require an incision of about 5 to 10cm to the<br />
skin over the site of the hernia in order to repair the<br />
abdominal defect. In the last decade, a new method<br />
of laparoscopic approach has been developed. All it<br />
requires are a 1cm incision at the belly button and two<br />
other 0.5cm puncture holes at the lower abdomen for<br />
the completion of the repair.<br />

How Is Laparoscopic Hernia<br />
Repair Performed?<br />
General anaesthesia is usually required and<br />
recommended. A 1cm incision is made at the lower<br />
border of the belly button for the insertion of a<br />
laparoscope connected to a camera and a light source.<br />
A space is then opened in the abdominal wall just in<br />
front of the peritoneal lining. This is the space in which<br />
the deficiency in the abdominal wall of the hernia can<br />
best be visualized. The image of the operative field is<br />
projected onto the TV monitor for the surgeon to conduct<br />
the surgery. Two slender instruments are put into the<br />
space via two puncture holes 0.5cm in size on the<br />
skin for operating. The hernia is then reduced under<br />
laparoscopic vision and a patch (artificial mesh)<br />
is put into the space to cover the hernia defect.<br />
On healing, the deficiency of the abdominal muscular<br />
wall is strengthened by the mesh to produce a sound<br />
hernia repair.
