Acknowledgement<br />
Registrations will be acknowledged via email with<br />
confirmation of your requirements according to your<br />
registration form, together with a tax invoice receipt.<br />
Bookings will only be confirmed when payment is received.<br />
Method of Payment<br />
• Payment of fees must accompany all registration forms.<br />
• All cheques, international bank drafts, money orders must<br />
be made payable to “<strong>Conference</strong> Action – All Together<br />
Better Health 5 <strong>Conference</strong>”.<br />
• Bank cheques and drafts must be drawn on a major<br />
branch of an Australian Bank in Australian dollars.<br />
• Register online at www.ATBH5.com.au if you are paying<br />
by credit card.<br />
• Credit cards accepted are Mastercard, Visa & American<br />
Express (please note Diners is NOT accepted).<br />
Cancellation Policy<br />
All cancellations or alterations should be put in writing to the<br />
ATBH5 <strong>Conference</strong> Secretariat. Please note that there is no<br />
refund on accommodation bookings.<br />
If cancellation is received in writing:<br />
• By 19 February 2010 A$100.00 cancellation fee will apply<br />
• By 5 March 2010 50% of monies paid<br />
• After 5 March 2010 No refund of monies paid<br />
