PART II: Research, Teaching, Leadership and Clinical Contributions<br />
Narrative Report<br />
I am a graduate of the MGH-<strong>McLean</strong> Adult Psychiatry Residency Program and the Partners<br />
Healthcare Addiction Psychiatry Fellowship. My clinical work is based out of <strong>McLean</strong> <strong>Hospital</strong><br />
in Belmont, Massachusetts. As medical director at The <strong>McLean</strong> Center at Fernside, I work with<br />
multidisciplinary teams to assess and treat residential patients with a broad range of substance<br />
abuse and co-morbid psychiatric disorders. Leadership and administrative duties include<br />
overseeing and supervising the daily medical operations and quality of the drug and alcohol<br />
abuse treatment service, participation in utilization management, risk management and<br />
continuous quality improvement activities and integration of this program’s services with the<br />
clinical, research, educational, fiscal and operational components of <strong>McLean</strong> <strong>Hospital</strong>. My<br />
research interests lie in developing innovative, evidence-based psychosocial treatments for dual<br />
diagnosis populations. I am currently a <strong>McLean</strong> <strong>Hospital</strong> Maria Lorenz Pope Research Fellow<br />
working to create and pilot a novel integrated treatment for patients with co-occurring borderline<br />

personality disorder and substance use disorders using elements of motivational interviewing<br />
techniques and mentalization. I am involved in teaching and supervision, currently developing<br />
and teaching a motivational interviewing curriculum for PGY-3 residents and working<br />
individually with residents and medical students to develop interviewing skills and supervising<br />
difficult substance abuse cases. I am the editor of the The Massachusetts General <strong>Hospital</strong> and<br />
<strong>McLean</strong> <strong>Hospital</strong> Residency Handbook of Psychiatry (2010), have designed a flexible<br />
curriculum to train clinicians in the effective use of Alcoholics Anonymous as an adjunct for<br />
patients with alcohol use disorders, and am a trainer for buprenorphine prescribing.<br />

C. Report of Clinical Activities<br />
1. Description of Clinical Practice<br />
Director, <strong>McLean</strong> Center at Fernside. Clinical duties include working with a<br />
multidisciplinary team to assess and treat residential patients with a broad range of<br />
substance abuse and co-morbid psychiatric disorders. Administrative duties include<br />
overseeing and supervising the daily medical operations and quality of the drug and<br />
alcohol abuse treatment service, participation in utilization management, risk<br />
management and continuous quality improvement activities and integration of this<br />
program’s services with the clinical, research, educational, fiscal and operational<br />
components of <strong>McLean</strong> <strong>Hospital</strong>. Administrative duties include responsibility for the<br />
development and implementation of all general, emergency, and personnel policies and<br />
procedures necessary to ensure a safe, productive and therapeutic program and working to ensure<br />
that all policies and procedures are designed to comply with standards set by the hospital and<br />
regulatory and accrediting agencies, over site of the business management and clerical functions<br />
with the Administrative Assistant/Secretary and was responsible for overseeing implementation<br />
of payroll systems, patient accounts, accounts payable, petty cash, and purchases for the program<br />
as well as monitoring expenses, revenues and allocation of resources and assisting in<br />
development of budgets for projected fiscal year in collaboration with the fiscal department of<br />
<strong>McLean</strong> <strong>Hospital</strong> and worked with the Vice President, Network Development at <strong>McLean</strong><br />
<strong>Hospital</strong> and <strong>McLean</strong> <strong>Hospital</strong> Department of Network Development to ensure that the program<br />
operates within the budget.<br />
Clinical responsibilities include direct supervision of the Clinicians/Case Managers and direct<br />
care staff including nurses and CRCs and completing performance evaluations for these<br />
employees as well as over site of development of annual training plan and its implementation to<br />
meet credentialing criteria and policy requirements and conducting training in relevant areas of<br />
expertise.<br />

Maria Lorenz Pope Research Fellow, <strong>McLean</strong> <strong>Hospital</strong>. Develop and manualize a treatment<br />
for co-occurring BPD and SUD that incorporates elements of empirically proven treatments,<br />
including Motivational Interviewing and standard relapse prevention counseling for SUD and<br />
Mentalization-based treatment for BPD. The development of the manual will followed by a<br />
piloting and feasibility study of eight patients with co-occurring BPD and SUD in an individual<br />
therapy setting. Primary outcome measurements will include treatment retention, patient<br />
satisfaction, therapist adherence, self report of substance use and borderline behaviors and results<br />
of urine drug screens and breathalyzer testing. At the completion of the study, we expect to have<br />
produced a manual for co-occurring BPD and SUD for use in a subsequent larger trial that will<br />
test the efficacy of this treatment.<br />
Private Outpatient Practice. Clinical duties include assessment and treatment of<br />
outpatients with a broad range of substance abuse and co-morbid psychiatric disorders.<br />
Psychotherapy patients are seen biweekly, weekly or twice-weekly. Psychopharmacology<br />
patients are seen twice a month, monthly or every three months.<br />
