We look forward to meeting you as a member of the Valencia College Summer 2013 Radiography Program. This
letter provides information regarding your mandatory orientation and other requirements you must complete by the
established deadlines. You will also find several documents that will assist you in completing the requirements.
Please check your Atlas e-mail regularly for information, as Atlas e-mail is the official form of communication
between you and your Valencia faculty and fellow students.
In order to reserve your place in the Summer 2013 Radiography Program, you must complete the tasks described in
this packet.
1. Attend Mandatory Program Orientation
Date: Friday, March 22, 2013
Formal Program Orientation: 10:30 a.m. – 12:30 p.m.
Location: West Campus, Building AHS 239
Parking: Parking lots A, B, or C (white lines only)
Professional dress is required (no shorts, jeans, etc.)
2. Immediately following your formal program orientation, the following services and vendors will be
available from 1:00 – 4:00 p.m. in the following locations:
HSB 228: FIT testing for your respirator mask will be provided during orientation
Please do NOT chew gum, drink or eat for at least 30 minutes prior to your FIT test. If you
must have something to drink, you may have ONLY WATER
Men should be clean shaven
Men and women with long or braided hair must be prepared to tie back
HSB 221: Centra Care for immunizations/titers on site for your convenience; a student pricing list is
attached.
Bring cash, checkbook, and credit/debit cards. The attached Disclosure of Costs for your program will help
you know how to plan.
3. Order your background check, fingerprinting, and drug testing
In this packet, please locate the Certified Background Instructions for submission of your background
check, fingerprinting, and drug testing.
Once you initiate your Certified Background account, you will be asked to select an Immunization/
Document Tracker that will outline the requirements you must meet prior to the deadline of April 19,
2013. The following fee is payable at the time the order is placed: $176. For a small processing fee,
you are offered the opportunity to make this payment in three increments.
You must complete the background check, fingerprinting and drug testing through
CertifiedBackground.com regardless of any other background check, fingerprinting or drug testing you
have completed previously.
It is essential to familiarize yourself with the information and attachments in this letter so you are ready for the
mandatory orientation on March 22, 2013.

About CertifiedProfile.com
CertifiedProfile is a secure platform that allows you to order your background check online. Once you have placed your order, you
may use your login to access additional features of CertifiedProfile, including document storage, portfolio builders and reference
tools. CertifiedProfile also allows you to upload any additional documents required by your school.
Order Summary
Required Personal Information - In addition to entering your full name and date of birth, you will be asked for your Social
Security Number, current address, phone number and e-mail address.
Drug Test (LabCorp) - Within 24-48 hours after you place your order, the electronic chain of custody form (echain) will be
placed directly into your CertifiedProfile account. This echain will explain where you need to go to complete your drug test.
Immunizations - Document trackers provide secure online storage for all of your important documents. At the end of the
online order process you will be prompted to upload specific documents required by your school for immunization, medical
or certification records.
Fingerprints - The online order process will guide you through the steps to complete a statewide Live Scan Fingerprint with
FDLE.
Payment Information - At the end of the online order process, you will be prompted to enter your Visa or Mastercard
information. Money orders are also accepted but will result in a $10 fee and an additional turn-around-time.

Your results will be posted directly to your CertifiedProfile account. You will be notified if there is any missing information needed in
order to process your order. Although 95% of background check results are completed within 3-5 business days, some results may
take longer. Your order will show as “In Process” until it has been completed in its entirety. Your school's administrator can also
securely view your results online with their unique username and password. Your fingerprint results are sent directly to the school
from the FDLE and are not viewable online.

Immunization Requirements
Measles (Rubeola)
-If you were born before 1956 you are exempt from this requirement.
-If you were born after 1956 there must be documentation of one of the following:
• 2 vaccinations
• Positive antibody titer for Measles (Rubeola)
Rubella
-If you were born before 1956 you are exempt from this requirement.
-If you were born after 1956 there must be documentation of one of the following:
• 1 vaccination
• Positive antibody titer for Rubella
Varicella (Chicken Pox)
-There must be documentation of one of the following:
• 2 vaccinations
• Positive antibody titer
• Medically documented history of disease
Hepatitis B
-There must be documentation of one of the following:
• 3 vaccinations
• Positive antibody titer
• Declination waiver
TB Skin Test
-There must be documentation of one of the following:
• 1 step TB Skin test
• Quantiferon Gold Blood test
• If positive results, provide a clear Chest X-Ray
Tetanus & Diphtheria (Td)
-Documentation of a Td booster within the past 10 years. CDC (2009) recommends at least one dose of Tdap (which includes
pertussis), also good for 10 years.
CPR Certification
-Must be the American Heart Association Healthcare Provider course. Copy must be front and back of the card. Certificates or
letters from provider are accepted temporarily until card arrives with a renewal date of 30 days.
Physical Examination
-Provide your physical exam form completed and signed by a medical professional.
N95 Fit Test
-Submit your certification indicating size, manufacturer and date.
