5310 SPECIALIZED TRANSPORTATION PROGRAM<br />
The 5310 Program improves access to transportation for elderly persons and persons<br />
with disabilities in the state <strong>of</strong> <strong>Oklahoma</strong>. The OKDHS Aging Services Division<br />
administers the 5310 Program. The Federal Transit Administration provides funding<br />
for vehicles and equipment.<br />
FREQUENTLY ASKED QUESTIONS<br />
• Who can use the 5310 Program?<br />
Any elderly person and person with a disability can use the program. However,<br />
contact your local transportation agency to find out their specific guidelines for<br />
individuals less than18 years <strong>of</strong> age.<br />
• How can the 5310 Program help me?<br />
The program provides transportation to various programs and services for<br />
medical, employment, education, shopping, and social needs. Contact your local<br />
transportation agency to see what type <strong>of</strong> service is available in your area.<br />
• How do I find 5310 Program services in my area?<br />
Services are available in most counties across the state. Check the map and listings<br />
in this book to find contact information for transportation agencies. Contact your<br />
local transportation agency to request services.<br />
• Is there a charge for 5310 Program services?<br />
The transportation service is free. However, the transportation agencies may accept<br />
donations.<br />
• What are my rights if I use the 5310 Program?<br />
<strong>Transportation</strong> agencies must provide reasonable Americans with Disabilities Act<br />
accommodations to all riders. Personal care attendants and service animals are<br />
allowed to accompany riders when needed.<br />
<strong>Transportation</strong> agencies cannot discriminate against any person based on age,<br />
gender, race, ethnicity, and/or national origin.<br />

5310 SPECIALIZED TRANSPORTATION PROGRAM<br />
NATURE OF THE PROGRAM<br />
This program purchases vehicles and other equipment to help private non-pr<strong>of</strong>it<br />
organizations and public agencies, under certain conditions, provide for the special<br />
needs <strong>of</strong> the elderly and persons with disabilities in urbanized and non-urbanized<br />
areas, where existing or proposed transportation services are not adequate. Applicant<br />
organizations must provide a matching fund percentage depending on the type<br />
<strong>of</strong> equipment that is ordered. The Federal Transit Administration will finance the<br />
remaining cost for approved projects.<br />
Awards will be in the form <strong>of</strong> vehicle(s) or other approved equipment. Type <strong>of</strong><br />
vehicle(s) awarded will meet the requirements <strong>of</strong> the Federal Transit Administration.<br />
Vehicles are purchased through statewide contracts. Title to vehicles is held by both<br />
the sub-grantee organization and OKDHS Aging Services Division.<br />
APPLICANT ELIGIBILITY<br />
Only private non-pr<strong>of</strong>it organizations incorporated within the State <strong>of</strong> <strong>Oklahoma</strong>,<br />
public agencies and associations determined by the Secretary <strong>of</strong> Treasury to be an<br />
organization described by 26 U.S.C. § 501(c) exempt from taxation under 26 U.S.C.<br />
§ 501(a) or determined under state law to be nonpr<strong>of</strong>it, are eligible. Documentation<br />
certifying the status <strong>of</strong> being a nonpr<strong>of</strong>it organization will need to accompany each<br />
application.<br />
Applications are accepted at any time.<br />
