<strong>Summary</strong> <strong>of</strong><br />

<strong>Support</strong> <strong>Order</strong><br />

What is the <strong>Summary</strong> <strong>of</strong> <strong>Support</strong> <strong>Order</strong><br />

(SOSO)?<br />

It is a form that summarizes a child support<br />

order. It is completed and presented <strong>to</strong> the<br />

judge by the at<strong>to</strong>rney or non-represented party<br />

in the child support court action.<br />

Why does a SOSO have <strong>to</strong> be completed?<br />

State law requires that a SOSO be submitted <strong>to</strong><br />

the judge with all court orders which establish<br />

paternity or establish, modify or enforce a child<br />

support obligation when services are not being<br />

provided by <strong>Oklahoma</strong> Child <strong>Support</strong> Services.<br />

[43 O.S. §120] These are referred <strong>to</strong> as “Non<br />

IV-D” child support accounts.<br />

Is the SOSO form filed with the court?<br />

No. The form is mailed or faxed <strong>to</strong> the OCSS<br />

Central Case Registry at P.O. Box 248843,<br />

<strong>Oklahoma</strong> City, OK<br />

73124-8843, fax (405) 522-8901.


Can a paternity or child support order be<br />

signed by a judge without a SOSO?<br />

No. A SOSO must accompany each paternity<br />

or child support order when services are not<br />

provided by OCSS.<br />

Where can I find a current version <strong>of</strong> the<br />

SOSO?<br />

A copy <strong>of</strong> the SOSO is available on the<br />

website at:<br />

A paper copy can be obtained by calling the<br />

CARE Cus<strong>to</strong>mer Call Center (see back page).<br />

Why does the form ask for Social Security<br />

numbers?<br />

Social Security numbers must be provided in<br />

order for child support payments <strong>to</strong> be<br />

identified with the correct case. [42 U.S.C. §<br />

666(a) (13)] The SOSO is not filed in the<br />

court file.


What information is required on the<br />

SOSO?<br />

• County <strong>of</strong> the child support order<br />

• Plaintiff and Defendant’s name<br />

• Case number<br />

• Type <strong>of</strong> order (temporary or final)<br />

• Kind<strong>of</strong>case<br />

• Whether there is an active<br />

protective order<br />

• Amount <strong>of</strong> child support order<br />

and the beginning and end date<br />

<strong>of</strong> the order<br />

• Employer name, address, and<br />

phone number<br />

• Full name, sex, date <strong>of</strong> birth,<br />

Social Security number (all 9<br />

digits) <strong>of</strong> the obligee (cus<strong>to</strong>dial<br />

person) and the obligor<br />

(noncus<strong>to</strong>dial person)<br />

• Address <strong>of</strong> record <strong>of</strong> the obligee<br />

and the obligor<br />

• Address where child support<br />

payments are <strong>to</strong> be mailed if<br />

different from home address<br />

• Date, name and phone number <strong>of</strong><br />

the person completing the form


What happens <strong>to</strong> the SOSO after it is<br />

prepared?<br />

Mail or fax the SOSO <strong>to</strong> the Central Case<br />

Registry, P.O. Box 248843, <strong>Oklahoma</strong> City,<br />

OK 73124-8843,<br />

fax (405) 522-8901.<br />

If the SOSO is not completed thoroughly,<br />

the Central Case Registry (CCR) will<br />

contact the preparer before a child support<br />

payment can be processed.<br />

The CCR will create a Non IV-D pass<br />

through child support account.<br />

The CCR is responsible for maintaining<br />

addresses <strong>of</strong> record for noncus<strong>to</strong>dial parents<br />

and cus<strong>to</strong>dial persons. The address is used<br />

for mailing out child support payments.<br />

Can either party obtain a record <strong>of</strong><br />

payments from OCSS?<br />

Yes. Child support payment records are<br />

available <strong>to</strong> the noncus<strong>to</strong>dial parent or the<br />

cus<strong>to</strong>dial person on the website at<br />

https://webpin.okdhs.org/webpin<br />

Contact the CARE Cus<strong>to</strong>mer Call Center at<br />

the phone numbers listed on the back page <strong>to</strong><br />

request a Record <strong>of</strong><br />

Payment or ID/PIN<br />

letter.


What are the benefits <strong>of</strong> accurately<br />

• Accounts are initiated<br />

promptly and payments are<br />

distributed in a timely manner<br />

<strong>to</strong> the cus<strong>to</strong>dial person<br />

• Payments are then received by<br />

either direct deposit or child<br />

support debit card<br />

• Electronic Funds Transfer is<br />

available for employers<br />

• Web pay is available for the<br />

noncus<strong>to</strong>dial person and<br />

employers<br />

• Payment records are accurate<br />

and available <strong>to</strong> an authorized<br />

person<br />

• Spousal support payments are<br />

processed accurately and<br />

promptly<br />
