Summary of Support Order What is the Summary of Support Order (SOSO)? It is a form that summarizes a child support order. It is completed and presented to the judge by the attorney or non-represented party in the child support court action. Why does a SOSO have to be completed? State law requires that a SOSO be submitted to the judge with all court orders which establish paternity or establish, modify or enforce a child support obligation when services are not being provided by Oklahoma Child Support Services. [43 O.S. §120] These are referred to as “Non IV-D” child support accounts. Is the SOSO form filed with the court? No. The form is mailed or faxed to the OCSS Central Case Registry at P.O. Box 248843, Oklahoma City, OK 73124-8843, fax (405) 522-8901.
Can a paternity or child support order be signed by a judge without a SOSO? No. A SOSO must accompany each paternity or child support order when services are not provided by OCSS. [43 O.S. § 118 D (I)] Where can I find a current version of the SOSO? A copy of the SOSO is available on the website at: www.okdhs.org/library/forms/OCSS/ A paper copy can be obtained by calling the CARE Customer Call Center (see back page). Why does the form ask for Social Security numbers? Social Security numbers must be provided in order for child support payments to be identified with the correct case. [42 U.S.C. § 666(a) (13)] The SOSO is not filed in the court file.
