– Application form PS 3615 for Permit Number<br />
No Fee<br />
• QBRM<br />
• Annual fee of $200<br />
– Application Form 6805 to the postmaster or manager, Business Mail<br />
Entry at the Post Office to which the QBRM pieces are to be returned.<br />
• CRM and MRM do not have any applications<br />
• PRM<br />
– Authorization through the district manager of Business Mail Entry at the<br />
office where the permit imprint account is held<br />
