The reports from the last two compliance audits by the certifying body are to confirm that<br />

the Applicant is managing quality issues in accordance with its QMS.<br />

Complete reports must be provided and must include all comments.<br />

Has the Applicant received any major non-con<strong>form</strong>ances from its certifying body in relation to<br />

its QMS over the last three years? If so, provide details and measures taken for corrective<br />

action and the outcome of these actions.<br />

GUIDE NOTE:<br />

Where system non-con<strong>form</strong>ances are reported the number, nature, and how the<br />

contractor took corrective action will be noted. All non-con<strong>form</strong>ances must be reported<br />

(unedited).<br />

4.9. Utilisation of Work Health and Safety (WHS) Management Systems<br />

GUIDE NOTE:<br />

Applicants that are accredited with the Office of the Federal Safety Commissioner under the<br />

Australian Government’s Building and Construction WHS Accreditation <strong>Scheme</strong> are not required to<br />

complete this Section 4.9.<br />

All prequalification categories<br />

Provide evidence of a system in place, including a signed copy of the Applicant’s corporate WHS<br />

policy statement and a sample or example WHS Plan.<br />
