Extended Rural
Cohort Program
Welcome
Congratulations on being part of an extraordinary and innovative program!
I hope you will find your time as part of the Northern Victoria Rural Medical
Education Network Extended Rural Cohort (NVRMEN ERC) exciting,
challenging, and most of all, rewarding.
Other Universities, such as Flinders, Harvard and the University of Washington
have run extended rural clinical education programs, for small numbers of
students. NVRMEN however, is the first multi-university program that brings
together expertise from two world-class medical schools, to train 60 students
per year in such a program.
Just like developing a space program that sends rockets and people into space,
we expect there may be a few challenges, and a few ‘course corrections’
needed on the way.
The fantastic part about being involved in an innovative program is that the
best people who have a sense of adventure (i.e. clinical teachers and students),
are doing their best to ensure everything works. And of course, where things
aren’t perfect, we’ll be working hard to make them better.
And, just like a rocket on course to the moon, we will need your feedback, early
and often, to ensure those course corrections for a smooth and successful
flight.
I look forward to working closely with all of you, as we go on this amazing
adventure together.
I know we all share the goal of helping you become outstanding doctors who
may be future rural doctors (and maybe even academic leaders for rural medical
education?).
The future really is yours and I look forward to sharing your experience and
seeing your success in your NVMEN ERC training.
Yours most sincerely,
Same curriculum, different delivery
Welcome to your community-based training year as part of your Extended Rural
Cohort (erC) experience.
A network of enthusiastic regional hospitals and general practices has joined with
the University of Melbourne to create the Northern Victoria Regional Medical
Education Network (nvrmen).
nvrmen is the partnership of rural and regional hospitals (‘hubs’) and general
practices (‘spokes’) which will be your training homes for the next 12 months.
You’ll be placed in the community setting, learning through General Practice (gp)
clinics, in regional and small rural hospitals, with visiting specialists and through
allied health services.
Students will learn from experienced, professional teachers in all settings. The
gp supervisors have been selected because of their educational interest and
experience, particularly in GP training. Hub educators have been appointed to
oversee the learning at each Hub. Hub education brings in a range of talent,
including specialists, GPs, and allied health and professional staff. GP registrars
also offer peer support in learning.
The day-to-day clinical curriculum is based upon ‘who walks in, or is, wheeled in
the door.’ Many studies have shown the powerful nature of this ‘real-life’ learning
opportunity, for both academic results, and attachment to future rural practice.
Examinations are the same as in traditional block rotation, and ERC students will
also complete mock exams at the end of Semester 10. Students will log their
tasks throughout the year in “U-Prime” a purpose built on-line education platform.
Students will spend 2 semesters in the one setting. where you will be supported
in the community by a range of mentors, academic and non-medical.
The ERC program works collaboratively with peer organisations including Bogong
gp training, the Department of Health Victoria and Monash University. Program
evaluation is continually sought and student feedback is valued in future planning.
Welcome to the ERC and enjoy the experience.
mansfield
Situated in the foothills of the Great Dividing
Range, Mansfield is surrounded by grazing land and
mountain forests, with the area well-known for its
stunning scenery.
The town centre has several historical buildings
dating back to its days as a stock route camp in the
1940s and a gold mining service centre beginning in
the 1850s. Mansfield is very much a tourist town,
featuring a wide range of accommodation and
vibrant commercial centre which caters for the many
visitors who then travel to the ski resorts of Mount
Buller or Mount Stirling via Merrijig.
Around 25 kilometres north of Mansfield on the
Midland Highway is Lake Nillahcootie which is a
manmade lake built in 1968 on the Broken River and
used for irrigation. Visitors to the lake enjoy attractive
picnic grounds, BBQs and a boat ramp, making
the lake suitable for fishing, swimming and boating
activities.
Lake Eildon and its surrounding national park lie
close to Mansfield, with numerous access points.
From the thrills of skiing, snowboarding, mountain
biking, four-wheel driving and horse riding to the
calmer pace of day spas, fishing, boating on Lake
Eildon, scenic drives and bushwalking, there is
something for everyone.
Guidelines for students
See also the RCS Handbook for Student Charter, Guidelines for Students and School Policies
PROFESSIONALISM
There are professional behaviours expected of
doctors that apply to student doctors as well.
Patients, fellow students, school staff, General
Practice and hospital staff all must be treated with
respect and dignity.
As part of your professionalism you should treat
Clinical School like a job, and our expectation is that
you attend all General Practice, Hub and hospital
scheduled clinical teaching and learning activities.
It is very important that you arrive on time and
therefore manage your transport and other logistics
to enable this.
Leave of Absence must be applied for in advance
in the normal way, through the Rural Clinical School
(see RCS Handbook or website - http://medicine.
unimelb.edu.au/assets/pdf/currentmd/forms/mms_
sloa_Form_v1.1.pdf
Your clothing should be neat and clean, without being
seductive or revealing. You must master the handling
of strong emotions in your patients, colleagues and
yourself. Impaired doctors are dangerous for patients.
Thus, if you are feeling incapable of performing as a
student, you need to recognise that, and ask for help,
whether this is for mental or physical health reasons,
substance abuse or personal reasons.
Confidentiality
As with hospital placements, students in small rural
communities will be told information by patients
and treating clinicians that may be personally
sensitive. The opportunities for possible breaches
of confidentiality are many - reception staff, hospital
staff and other patients. Students are expected to
uphold the rights of patients to have their personal
information kept restricted to the treating clinicians
and other members of the health team on a “need to
know” basis. Wilful disregard of these guidelines will
be viewed very seriously.
LMS details
The primary method of communication used by the
Rural Clinical School for students is LMS. Messages
will include student announcements about school
accommodation facilities and non-urgent timetable
changes. Students are expected to check LMS
daily. lms will be available to supervisors to allow
some access to curriculum areas taught by the GP
supervisors.
‘UPrime’
UPRIME is an online tool where you and your
supervisors will be required to sign off on learning
tasks and procedures. Students will receive a log
in to this program, along with user instructions and
expectations about how you are expected to use this
tool.
Assessment and examinations
The timetable of assessment as available at the
time of printing is shown below - this is a draft
document but major changes are not expected.
Of major importance is that ERC students will have
exactly the same examinations and assessments as
other students - the only difference is in the timing.
Please note: Contrary to some advice, Intern Marks
provided by the University of Melbourne to the PMCV
do not include any component of Semester 10-11,
being largely based on Semester 8 and 9 rankings.
Thus ERC students will be at no disadvantage in the
Intern ranking system.
Vocational selective
The purpose of the “metro” selective is to give ERC
students an opportunity to have an experience within
the metropolitan Melbourne system as an “option”
if they have a special interest, or want to experience
a particular hospital. It might also be thought of as a
(metro) selective. In general, it will have to be in a
non-core area for capacity reasons.
It takes advantage of the 5-week “gap” that occurs
since students have already completed GP.
There is no requirement that it be at a metropolitan
site per se, but it must be in the Melbourne Medical
School system and thus is not an “elective”.
