their student audience. The submission of this audition form and the signatures below indicate that all
persons have read and agree to the rules and policies described in the Talent Show Information found on
the Oak-Land website. The undersigned students and parents understand that consequences for not
adhering to the rules and policies described, to any school behavior policy, or to the spirit of this schoolsponsored
activity could include removal from the show and appropriate disciplinary action in accordance
with school discipline policy.
