What happens during an assessment?<br />

The care co-ordinator will meet with you to:<br />

<br />

ask you questi<strong>on</strong>s about any difficulties you are having<br />

find out what you would like to do<br />

discuss what support is available to you<br />

The care co-ordinator will also talk to other workers involved in your<br />

care, e.g. doctors, occupati<strong>on</strong>al therapists and support workers. A<br />

report is then written to say what your needs are and what is going to<br />

be d<strong>on</strong>e to support you. This is called a care plan.<br />

You will receive a written care plan which will provide informati<strong>on</strong><br />

about what services you will receive and who will be resp<strong>on</strong>sible for<br />

making sure you receive these services.<br />

Who is involved in an assessment?<br />

You will be involved in planning your care with your care<br />

co-ordinator and other members of the team who are helping you.<br />

This may involve the c<strong>on</strong>sultant psychiatrist, your support worker,<br />

occupati<strong>on</strong>al therapist, ward/day hospital staff and you may wish to<br />

involve a carer, family members or an independent advocate.<br />

Care co-ordinati<strong>on</strong> standards you can expect<br />

We will provide people receiving care co-ordinati<strong>on</strong> with a<br />

copy of a written care plan<br />

The care plan will be regularly reviewed by the pers<strong>on</strong>’s care<br />

co-ordinator.<br />

You will receive a copy of a written care plan which will be<br />

agreed <strong>on</strong> your discharge from hospital. This will set out the<br />

care to be provided and identify the care co-ordinator.<br />

We will offer all carers who provide regular and substantial<br />

care for a pers<strong>on</strong> receiving enhanced care co-ordinati<strong>on</strong> an<br />

assessment of their needs and a support plan, which will be<br />

reviewed every year.<br />

To give you more involvement in how the service is<br />

planned and delivered we want to:<br />

make it easier for you to get the service you need.<br />

manage the service in partnership to ensure we help people<br />

to move through services easily<br />

The review meetings<br />

We will look at your care plan at meetings. These meetings are<br />

called “reviews”. They are held so that we can discuss with you any<br />

areas of c<strong>on</strong>cern you have and the progress you are making. If,<br />

following a review, a change is made to the services you receive you<br />

will be provided with an up-to-date care plan.<br />

are developed and provided.
