This Telephone Flow Chart is designed to
assist individuals who may answer the
telephone when a caller makes an initial
inquiry to seek information or services.
The “answerer” begins by helping the caller
out with the question they called about,
then, if the caller is open to talking about
anything beyond his/her initial issue, go to
the Flow Chart below. This Flow Chart will
take the answerer through a brief series
of questions. Depending on the caller’s
response, resources can be suggested
which are located in this Directory.

SERVICES
An adult day care center provides
services generally Monday
through Friday during normal
business hours in a group setting
to adults who need assistance
with the activities of daily living
and supervision. Participants
in adult day care may receive
assistance with personal cares,
supervision, meals, medication
administration, and may
participate in various social
activities and community outings.
In Wisconsin, adult day care
centers are not licensed, but may
be certified. Adult day care may
be provided in family homes, free
standing centers, and multi-use
facilities such as churches, schools
and senior centers.

Wisconsin Elder Abuse Law
Chapter 46.90
DEFINITION: An elder person, age 60 years or
older, who may be subject to abuse, material abuse,
neglect or self-neglect as defined below:
1. Physical Abuse means “willful infliction
on an elder person of physical pain or injury or
unreasonable confinement.” This could include direct
aggressive behavior such as bodily assault, sexual
abuse, physical restraint, torture and imprisonment.
2. Material Abuse means “the misuse of an
elder person’s property or financial resources.” This
would involve misuse, misappropriation of money,
valuables or property; denial of right to access or
control of personal funds, or interference in financial
decisions, passive neglect of financial affairs or
failure to intervene in financial mismanagement or
exploitation.
3. Neglect means “a significant danger to an
elder person’s physical or mental health because
the person who takes care of the older person is
unable or fails to provide adequate food, shelter,
clothing, medical or dental care.” This would include
neglecting, withholding or refusing to permit
others to provide food, clothing, shelter, medication,
medical attention, or personal care and neglectfully
permitting or deliberately causing exposure to
unsafe, unsanitary, or unhealthful conditions.
4. Self-Neglect means “a significant danger to an
elder person’s physical or mental health because
the elder person is responsible for his or her own
care, but is unable to provide adequate food, shelter,
clothing, medical or dental care.”
I. REFERRAL: Cases of elder abuse need to be
referred to the Winnebago County Department of
Human Services, Crisis Intervention/Adult Access

A social worker will be assigned to assess the
situation.
II. DISPOSITION: There are four basic types of
situations:
1. The elder person who consents to assessment
and service delivery and who appears to be mentally
competent.
2. The elder person who refuses assessment and
service delivery and who appears to be mentally
competent.
3. The elder person who (regardless of his or her
degree of cooperation) appears to lack sufficient
mental capacity to make decisions regarding his/her
own care or an elder person who has a legal guardian.
4. The elderly person is in danger of immediate
and grievous bodily harm regardless of competency
(emergency situations).
LIMITATIONS: Competent adults have the legal
right to refuse assessment and/or services regardless
of the opinion of professionals, family, friends,
neighbors and the community at large.

Information
In Winnebago County, there is a program
called the Community Options Program.
This program is administered through the
Winnebago County Department of Human
Services. All people that might be at risk of not
being able to live independently in their own
home, are eligible to receive a Long Term Care
Assessment/Consultation.
To receive an Assessment/Consultation, call
Winnebago County Human Services Department
Crisis Intervention/Adult Access Team at
920-236-4615 Oshkosh or 920-727-2882 Fox Cities
Monday through Friday, 8:00 a.m. -4:30 p.m.
After normal business hours, there is a Crisis
Hotline where assistance and information can be
received; that number is 920-233-7707 (Oshkosh)
and 920-722-7707 (Fox Cities).
The Long Term Care Assessment/Consultation will be
conducted by professionals that will work with loved ones in
determining what needs the person being assessed has and
will offer recommendations that can be followed to meet the
person’s needs. The following listed services may be those
where assistance is needed.
Personal Cares: bathing, dressing, exercise, grooming,
shaving, oral hygiene
Household Chores: cleaning, meal preparation, shopping,
laundry, money management, and socialization
Relief Help — Respite: adult day care (half, whole day,
week), in home staff (temporary or constant provider, or
week at CBRF)
Information — Connections: adult foster care, counseling,
funding, Life Line, nursing home consultation, physical
therapy, transportation, case management, education, housing,
Meals on Wheels, protective services, support groups,
and mobility aids
The Winnebago County Department of Human Services
also has a multitude of written materials and videos available
for loan on caregiver issues.
If a person has financial resources to pay for needed services,
the developed assessment plan will guide the family
to arrange needed services.
If the person does not have financial resources to pay for
needed services, there may be finances available to assist
these needs; however, there may be a waiting list to receive
financial assistance. It is important to receive the Long Term
Care Assessment/Consultation and get on a waiting list to
eventually receive financial assistance to meet needs.
The Long Term Care Assessment/Consultation can also be
done at a time of crisis, by hospital discharge planners, or by
home health care agencies.
Care coordination/
care management
Once needs have been determined,
whether as a result of the Long Term
Care Assessment or as families become
aware of needs, there is a need
to become more knowledgeable of
specific services. There is a wide array
of agencies that can be contacted.
Many families will opt to coordinate
and provide needed care themselves;
others will utilize services available in
the community and others will combine
utilizing available services and
provide some care themselves. This is
a personal decision for the family.
Many people needing assessments
and/or services do not have family
or family living nearby or family who
are willing and/or able to provide
assistance. The individual/family may
also choose to not make contact with
Winnebago County Department of
Human Services. The following agencies,
for a fee, will provide coordination
and other services privately in
Winnebago County. This type of service
is available throughout the country
and may bring piece of mind to
long distance caregivers or to people
who have no one to advocate/assist in
setting up needed services.

donate comprehensive dental care
to people of all ages who, because
of a serious disability, advanced
age, or medical problems, do
not have enough income to pay
for needed dental care. There is
generally no cost to qualifying
individuals, however, people in
a position to pay for part of their
care may be encouraged to do
so, especially when laboratory
work is involved. Call the Program
Coordinator for an application.
If the application is approved,
the referral coordinator will
share the information about the
person tentatively accepted with
a volunteer dentist, then you will
be notified of the dentist’s name
and phone number and you will
be responsible for setting up an

Relay System
The Wisconsin Relay system
provides the vital link for effective
telephone communication
between people who use a TTY
and those who use a standard
telephone. A TTY (also called a
TDD, TT, or text telephone) is a
device that allows people who can
not hear and/or speak to type and
read their telephone conversation.
Calls are relayed by highly trained
operators called Communication
Assistants (CA’s) who acts as
the “invisible link” between two
callers. During a relay call, a CA
will voice everything typed by

this way, the relay allows people

provides education, support and resources to family

the fourth Tuesday of every month at 1:30 p.m. at the

What is Respite care?
Respite care is temporary relief from duties for caregivers.
It may be for a few hours or for a few days and
can be provided in-home, at an adult day care, or in a
residential setting such as an assisted living or nursing

One of the most difficult decisions a
person may have to make is to move
from his or her home to a new living
environment. Some fully embrace
moving to a new situation where they no longer
have to worry about making and paying for
home repairs, mowing the lawn, shoveling snow
and going up and down stairs. For others, the
thought of change is difficult and frightening
and gets put off.
Changes may eventually need to be made
with the involvement of family or a community
agency.
This information is made available so that
people contemplating change can learn more
about what alternatives are available. There
may be more alternatives available; those listed
provided information for this directory. Some
of the facilities listed in this directory have
waiting lists. It is very important that you make
arrangements to get on a facility’s waiting list if
you desire to live there.
Remember, when looking for housing, find
information on the facilities you are interested
in. Make a list of questions that pertain to your
needs. Visit the facility, and ask your questions
so that you have all of the information you need
to know if this is a good choice for you.
The first part of the Directory gives definitions
of the different types of housing.
What is a Residential
Care Apartment
Complex?
Residential Care Apartment Complexes
(RCACs) are a form of housing
available to seniors in Wisconsin
designed to provide another choice
for long-term care. All facilities are
required to provide apartments with
a kitchen, living area, bedroom and
bathroom. In addition, RCACs must
be equipped to provide up to 28
hours per week of services designed
to meet individual tenants’ needs.
These types of services may
include (but are not limited to) help
with bathing, dressing and grooming,
nursing services such as medication
set-up and monitoring, and help
with laundry and housekeeping.
This provision allows consumers
to purchase the service they want,
with the flexibility to increase services
as their care needs increase in
the future.
How do RCACs differ
from Nursing Homes and
Assisted Living?
States are responsible for regulating
long-term care facilities. While
the commonly accepted or generic
term for facilities other than nursing
homes is “assisted living,” different
states have chosen a variety of
names. In Wisconsin there are really
three different kinds of assisted living
facilities.
These are (1) RCACs; (2) Community
Based Residential Facilities
(CBRFs); and (3) Adult Family
Homes. Different regulations apply
to each.
A CBRF is generally a facility
where individuals have either a
separate or shared bedroom, but
definitely share common living
spaces such as the living room,
dining room and kitchen. They may
share bathrooms as well.
CBRFs are very well-regulated and
monitored by the State of Wisconsin.
For example, CBRFs are required
to do background checks on their
employees before hiring them.
Their staff must undergo certain
types of training for specific numbers
of hours on topics such as first
aid, fire safety, medication administration,
caring for the elderly and
other areas.
The state inspects all CBRFs when
they first apply for a license and then
at least once every subsequent year.
The state also sends out someone to
investigate if anyone files a complaint
about care or services in the
CBRF.
