STEP-BY-STEP GUIDE TO THE APPLICATION I n s t ructions for certain questions on the application are given below, with numbers corresponding to the number of the question on the application. Please read care f u l l y, follow all directions, and type or print all inform a t i o n except your signature . 2 Applicant Social Security Number Your Social Security Number is used to coordinate the collection of information for all your student records. Authority to collect the Social Security Number is granted under Section 355 of the New York Education Law. Your response, however, is voluntary. Please double check the number for accuracy. 9a – 9c Residency These questions concern your residency for tuition purposes as well as eligibility for the Educational Opportunity Program (EOP). At the State-operated campuses (University Centers, University Colleges and Technology Colleges), students are generally considered New York State residents if they have established their domicile in New York State for at least twelve months prior to the last day of the registration period. Generally, individuals who have maintained their domicile in New York for a period of less than twelve months prior to the end of registration are presumed to be out-of-state residents and are not eligible for the resident tuition rate. Individuals who are financially dependent and whose custodial parent(s) live in a State other than New York are generally not eligible for the resident tuition rate. Individuals do not meet the twelve-month residency requirement if domiciled in New York State for the sole purpose of attending college. Generally, only U.S. citizens (question 7) and permanent residents are eligible for the New York State tuition rate (see page A11). Dependent students of divorced or legally separated parents may acquire a New York State domicile if the custodial parent is a New York State resident or if the student resides with a non-custodial parent who is a New York State resident and the student intends to continue to reside with that parent throughout their attendance at SUNY. Please direct your questions concerning state residency to the campus offices of student accounts. 10 Name and Address of Parent or Guardian If your parents share custody and you would like both parents to receive official correspondence, you should list two names in the space provided. Please list the address of the second parent here. 11 Alumni in the Immediate Family Information gathered from this question does not affect admissions decisions. 12 Family Income Range Indicate the letter or number corresponding to your family’s gross income, as reported on the federal income tax form. If your parents are separated or divorced, you should report the income of the household in which you reside for most of the year. If your situation is described by the letters S, T, or W above, enter one of those codes rather than a letter or number indicating an income range. Please note: Your answers here will not impact financial aid decisions. However, if you wish to be considered as an EOP applicant, you must answer questions 9a, 9b, 12 and 13. W You are a ward of the state or a county. 13 Number of Dependents in Family Please include yourself and head of household in the total number of persons dependent for support upon your parents, your legal guardian, or you. 15 – 16b Diversity/Disabilities Your responses to Questions 15, 16a, and 16b on the attached application are strictly voluntary. In the event you choose to answer these questions, the State University of New York will keep your responses confidential and will not use the information provided in a discriminatory manner. The failure to respond to these questions will not subject you to any adverse treatment. The State University seeks to enroll students of diverse racial and ethnic heritage and students with disabilities. 19a – 19b Felony/Dismissal Information A felony in New York State law is defined as a crime for which more than one year in prison may be imposed. The felony question applies if you have been convicted as an adult. If you have been adjudicated as having juvenile delinquent or youthful offender status, you are required to respond to the felony question 19a by indicating a response of no. Suspension is defined as a sanction imposed for disciplinary reasons that results in a student leaving school for a fixed time period, less than permanently. Dismissal from a college for disciplinary reasons is defined as permanent separation from an institution of higher education on the basis of conduct or behavior. An affirmative response to either question will not automatically prevent admission, but you will be asked by your college choices to provide additional information. This information will be reviewed by a campus committee to ensure campus safety. For private colleges in New York, enter “NY” in the two spaces provided. For out-of-state colleges, enter the respective state code. For Puerto Rico enter “PR.” For Canadian and other foreign colleges, enter “FR.” 43 – 44 Total Credits/GPA for Transfer Students For a college where you are currently enrolled, please enter all earned credits, both past and in progress, and enter your current GPA (grade point average). 45 College CEEB Codes College CEEB codes can be found through the College Board (collegeboard.com) or at su n y . e d u / student/guidebook/tables.cfm. 46 Additional College Information If you attended more than three colleges, for each, on a separate sheet of paper, please answer questions 40-45. 51a – 51b Temporary Address If you are away at college or attending a secondary school away from home, and wish to receive mail there temporarily, you should provide the address and indicate the date mail should revert to your permanent address. 53 Student Support Services Enter the letters corresponding to campus services you would like, if available, at your college choices. A Improving mathematical skills B Improving reading skills C Improving writing skills D Developing good study techniques E Personal, career or educational counseling F Finding part-time jobs G Locating off-campus housing 54 Educational Opportunity Program (EOP) – New York State residents only The EOP program is designed for students who need special academic and financial assistance. Please consult with your college advisor to determine if you qualify and to which campuses you should apply. You will be asked to complete additional application materials and financial aid forms by your college choices. Be sure to meet all deadlines. For a full program description and eligibility criteria, see page A10. 55 Early Decision Program The Early Decision Program is for fall freshmen and can be used for one college choice only. The application includes a request for a parent and counselor signature in addition to the student’s signature indicating an understanding of the Early Decision commitment and agreement to abide by its terms. Participants in the Early Decision Program are Brockport, Buffalo State College, University at Buffalo, College of Ceramics, Cortland, Environmental Science & Forestry, Fredonia, Geneseo, Maritime, Old Westbury, Oneonta, Oswego, Plattsburgh, Potsdam, and Purchase (except for fine and applied arts programs), and SUNYIT-Utica/Rome. Please note that you cannot apply for Early Decision and EOP at the same time. To apply for Early Decision, your application must arrive by November 1. The campus will notify you of its decision by December 15. If you are accepted Early Decision, you will be asked to make a tuition deposit by January 15 and withdraw your applications at all other campuses. Note: Early Action Early action is a non-binding early application/notification program for fall freshmen available only at PAGE A8 OF THE APPLICATION You may apply to as many as four State University college choices on this application. Use additional forms for additional choices. Please complete all information for each college choice. Make checks payable to “SUNY APC” at $40 per college choice (i.e., $80 for two choices, $120 for three, $160 for four, etc). You may also pay by MasterCard or Visa. Campus Code Whenever possible, spell out the name of the college and avoid abbreviations. Please be sure to enter the right code for campuses which share similar names, such as Buffalo State College (Code 33) and the University at Buffalo (Code 20); Alfred State College (Code 91) and the College of Ceramics at Alfred University (Code 30); Genesee Community College (Code 81) and the College at Geneseo (Code 36). Curriculum Code Please refer to the curriculum index at the back of the A p p l i c a t i o n Viewbook and specify a curriculum code for each college choice. You Albany, Binghamton, Stony Brook, New Paltz and F.I.T. If you are accepted Early Action, you need not withdraw your applications at other institutions until May 1. To apply Early Action, your application should reach us by November 15. The campuses will notify you of their decisions by January 1. If accepted Early Action, you will have until May 1 to finalize your enrollment decision. Please indicate your desire to apply Early Action where indicated in the “Choosing Colleges” section on page A8. may also indicate that you do not wish to declare a major. Please note that the health science centers and most community colleges, including F.I.T., require that you declare a major. Early Action Please check here if you wish to apply Early Action at Albany, Binghamton, Stony Brook, New Paltz or F.I.T. (see note above). Please note that you cannot apply for Early Action and EOP at the same time. Special Campus Project A campus may request that you use a local code in this space. If you have completed one or more tech prep courses, please insert “TPREP” in this space. Joint Admissions Joint admissions programs guarantee SUNY students graduating from two-year colleges admission to parallel programs in four-year colleges, subject to certain requirements.
