These practical <strong>and</strong> hygienic benefits <strong>of</strong> keep<strong>in</strong>g ur<strong>in</strong>e <strong>and</strong> faeces apart have led to <strong>the</strong><br />

c<strong>on</strong>clusi<strong>on</strong> that we should aim for ur<strong>in</strong>e diversi<strong>on</strong> <strong>in</strong> all dry sanitati<strong>on</strong> systems. It may also be<br />

beneficial to supplement waterborne sanitati<strong>on</strong> systems with ur<strong>in</strong>e diversi<strong>on</strong> to allow <strong>the</strong> use<br />

<strong>of</strong> ur<strong>in</strong>e as a fertilizer <strong>and</strong> to decrease <strong>the</strong> envir<strong>on</strong>mental effects from nutrients <strong>in</strong> toilet waste,<br />

i.e. eutrophicati<strong>on</strong>. Source-separat<strong>in</strong>g (divert<strong>in</strong>g) systems have <strong>the</strong>refore been identified as<br />

part <strong>of</strong> susta<strong>in</strong>able development <strong>and</strong> substantial research is currently carried out <strong>in</strong> several<br />

countries, <strong>of</strong> which Sweden has been <strong>on</strong>e <strong>of</strong> <strong>the</strong> first.<br />

excreta, it is possible to close <strong>the</strong> nutrient loop.<br />

A ma<strong>in</strong> aspect is that <strong>the</strong> use <strong>of</strong> excreta should not result <strong>in</strong> enhanced disease transmissi<strong>on</strong> <strong>and</strong> an<br />

<strong>in</strong>creased number <strong>of</strong> <strong>in</strong>fecti<strong>on</strong>s <strong>in</strong> human populati<strong>on</strong>s. The current <strong>EcoSanRes</strong> <str<strong>on</strong>g>Guidel<strong>in</strong>es</str<strong>on</strong>g> for <strong>the</strong><br />

h<strong>and</strong>l<strong>in</strong>g <strong>and</strong> use <strong>of</strong> collected ur<strong>in</strong>e <strong>and</strong> faeces <strong>the</strong>refore aims to m<strong>in</strong>imize <strong>the</strong> risk for transmissi<strong>on</strong><br />

<strong>of</strong> <strong>in</strong>fectious diseases that potentially can occur through ur<strong>in</strong>e <strong>and</strong>/or faeces.<br />

Disease-caus<strong>in</strong>g microorganisms <strong>in</strong> excreta<br />

Occurrence <strong>of</strong> disease-caus<strong>in</strong>g organisms <strong>in</strong> human excreta is <strong>the</strong> result <strong>of</strong> <strong>in</strong>fecti<strong>on</strong> <strong>in</strong><br />

<strong>in</strong>dividuals. Such <strong>in</strong>fecti<strong>on</strong>s do not necessarily manifest with cl<strong>in</strong>ical symptoms, but will lead<br />

to an excreti<strong>on</strong> <strong>of</strong> <strong>the</strong> pathogens <strong>in</strong> questi<strong>on</strong>. For organisms <strong>in</strong>fect<strong>in</strong>g <strong>the</strong> gastro<strong>in</strong>test<strong>in</strong>al track,<br />

this excreti<strong>on</strong> is ma<strong>in</strong>ly through faeces.<br />

The prevalence <strong>of</strong> <strong>in</strong>fecti<strong>on</strong>s mirrors <strong>the</strong> hygienic situati<strong>on</strong> <strong>in</strong> a society. Infecti<strong>on</strong>s are always<br />

an excepti<strong>on</strong> <strong>and</strong> not a general situati<strong>on</strong> for an <strong>in</strong>dividual. Infecti<strong>on</strong>s <strong>of</strong> <strong>in</strong>dividuals may, <strong>in</strong> rare<br />

cases, be chr<strong>on</strong>ic, for bacterial <strong>and</strong> viral diseases. The <strong>in</strong>dividuals are <strong>the</strong>n called “carriers”.<br />

Parasitic worms (helm<strong>in</strong>ths) may establish <strong>the</strong>mselves for l<strong>on</strong>g periods <strong>in</strong> <strong>the</strong> human body <strong>and</strong><br />

have a high prevalence rate <strong>in</strong> societies with unsanitary c<strong>on</strong>diti<strong>on</strong>s.<br />

An <strong>in</strong>dividual will normally excrete large amounts <strong>of</strong> microorganisms <strong>in</strong> faecal material.<br />

The numbers are <strong>in</strong> <strong>the</strong> range <strong>of</strong> 10 11 -10 13 /g. These saprophytic organisms are normally <strong>of</strong> no<br />

health c<strong>on</strong>cern. <strong>Ur<strong>in</strong>e</strong> is normally sterile <strong>in</strong> <strong>the</strong> ur<strong>in</strong>e bladder, but “pick up” organisms that<br />

occur <strong>in</strong> <strong>the</strong> lower parts <strong>of</strong> <strong>the</strong> ur<strong>in</strong>ary tract. Thus, a c<strong>on</strong>tent <strong>of</strong> 10 3 organisms/ml <strong>of</strong> ur<strong>in</strong>e is not<br />

<strong>in</strong>dicative <strong>of</strong> an <strong>in</strong>fecti<strong>on</strong>. These saprophytic organisms are also generally harmless.<br />

If a disease-caus<strong>in</strong>g organism <strong>in</strong>fects a pers<strong>on</strong>, <strong>the</strong> cl<strong>in</strong>ical manifestati<strong>on</strong>s are governed by<br />

factors related to <strong>the</strong> organism <strong>in</strong> questi<strong>on</strong> <strong>and</strong> by factors related to <strong>the</strong> <strong>in</strong>fected <strong>in</strong>dividual.<br />

Most <strong>of</strong> <strong>the</strong> disease-caus<strong>in</strong>g organisms <strong>of</strong> c<strong>on</strong>cern are excreted, <strong>in</strong> variable numbers, <strong>in</strong> faeces,<br />

but a few also through <strong>the</strong> ur<strong>in</strong>e. The likelihood <strong>of</strong> <strong>the</strong>m result<strong>in</strong>g <strong>in</strong> new <strong>in</strong>fecti<strong>on</strong>s <strong>in</strong> o<strong>the</strong>r<br />
