What is the Homebound Program?
General Education Homebound
placement is a service provided for
students physically unable to attend
school due to a medical problem. A
homebound student will receive
instruction by a certified general
education teacher for a minimum of four
hours a week for a full day student. The
homebound teacher and classroom
teacher(s) collaborate to provide
appropriate curriculum assignments to
enable the student to make a positive,
smooth transition upon return to school.
Who is Eligible for Homebound
Services?
A student eligible for homebound
services must be enrolled in a public
school within the boundaries of Plano
Independent School District and,
because of a medical condition, is
unable to attend school.
A physician’s medical report
recommending the student be confined
to his or her home for a minimum of
four consecutive weeks is mandatory for
homebound eligibility.
Examples of physical reasons a student
might require homebound services
include chemotherapy, serious chronic
health problems, or a student recovering
from surgery. If a student is contagious,
homebound services will not be
provided until the physician designates
it is safe for the staff to interact with the
student.
Are Homebound Services Free?
Yes, since this is a public school
program, the family is not required to
pay for the service.
How are Homebound Services
Initiated?
A physician, parent, or student may
request this service by contacting the
school counselor. A homebound referral
will be completed. A form will be
provided for the physician to complete,
and return to school. Upon receiving the
physician’s medical report, the campus
assessment team will start the process to
initiate services. Parents will be asked
to provide consent to release
confidential information and medical
information.
Do Any Forms Need to be Returned
to School?
Yes, the following forms must be
completed and returned:
All forms for the initial referral
The physician’s medical report
Medical release prior to returning to
school
Restrictions of Homebound Services
Homebound students may be allowed to
attend school functions with approval
from the school administration.
Students are not allowed to be employed
while receiving homebound services.
Parent Responsibilities
Arrangements must be made in advance
for a responsible adult to be present in
the home during each session. This is for
the safety of the student as well as the
teacher. If an adult is not present, the
teacher will conclude the session and the
student will be counted absent.
If the student or someone in the home
contracts a contagious disease,
instruction will not take place. The
parent must contact the homebound
teacher so sessions can be rescheduled
or different arrangements made.
Arrangements should be made to
schedule doctor’s appointments around
homebound sessions whenever possible.
Since a student receives credit for 5 days
of attendance with 4 hours of
homebound services, an absence from a
homebound session could mean that the
student is counted absent for 2 1/2 days.
Student Responsibilities
The student must complete the
assignments left by the homebound
teacher each session. It is suggested that
the student spend three hours each day

on school work. The student should be
organized and ready for instruction each
session.
How Long Does it Take Before the
Homebound Teacher Makes the First
Visit?
After the paperwork is completed and
received by all parties, the campus
assessment team meeting will be held.
At that meeting a convenient time to
begin services will be agreed upon by
the parents and teacher. Services will
begin within a week after the campus
assessment team meeting.
Teacher Responsibilities
The classroom teacher and the
homebound teacher work
collaboratively to ensure the student’s
general education program is being
followed.
Classroom Teachers:
Supply assignments, worksheets,
materials and tests to the homebound
teacher. Each school has a
designated area to place homebound
materials.
Evaluate student’s work, assign
grades and return graded
assignments to student’s homebound
teacher weekly.
Assist in making the student’s
transition from homebound to school
a positive one.
Homebound Teachers:
Provide instruction in the home or
hospital for the student to gain skills
on weekly assignments.
Administer all exams.
Inform the teachers of student’s
progress and discuss problems.
Return completed work to school on
a weekly basis.
Report attendance to student records.
