What is the <strong>Homebound</strong> Program?<br />
General Education <strong>Homebound</strong><br />
placement is a service provided for<br />
students physically unable to attend<br />
school due to a medical problem. A<br />
homebound student will receive<br />
instruction by a certified general<br />
education teacher for a minimum of four<br />
hours a week for a full day student. The<br />
homebound teacher and classroom<br />
teacher(s) collaborate to provide<br />
appropriate curriculum assignments to<br />
enable the student to make a positive,<br />
smooth transition upon return to school.<br />
Who is Eligible for <strong>Homebound</strong><br />
Services?<br />
A student eligible for homebound<br />
services must be enrolled in a public<br />
school within the boundaries of <strong>Plano</strong><br />
<strong>Independent</strong> School District and,<br />
because of a medical condition, is<br />
unable to attend school.<br />
A physician’s medical report<br />
recommending the student be confined<br />
to his or her home for a minimum of<br />
four consecutive weeks is mandatory for<br />
homebound eligibility.<br />
Examples of physical reasons a student<br />
might require homebound services<br />
include chemotherapy, serious chronic<br />
health problems, or a student recovering<br />
from surgery. If a student is contagious,<br />
homebound services will not be<br />
provided until the physician designates<br />
it is safe for the staff to interact with the<br />
student.<br />
Are <strong>Homebound</strong> Services Free?<br />
Yes, since this is a public school<br />
program, the family is not required to<br />
pay for the service.<br />
How are <strong>Homebound</strong> Services<br />
Initiated?<br />
A physician, parent, or student may<br />
request this service by contacting the<br />
school counselor. A homebound referral<br />
will be completed. A form will be<br />
provided for the physician to complete,<br />
and return to school. Upon receiving the<br />
physician’s medical report, the campus<br />
assessment team will start the process to<br />
initiate services. Parents will be asked<br />
to provide consent to release<br />
confidential information and medical<br />
information.<br />
Do Any Forms Need to be Returned<br />
to School?<br />
Yes, the following forms must be<br />
completed and returned:<br />

The physician’s medical report<br />
Medical release prior to returning to<br />
school<br />
Restrictions of <strong>Homebound</strong> Services<br />
<strong>Homebound</strong> students may be allowed to<br />
attend school functions with approval<br />
from the school administration.<br />
Students are not allowed to be employed<br />
while receiving homebound services.<br />
Parent Responsibilities<br />
Arrangements must be made in advance<br />
for a responsible adult to be present in<br />
the home during each session. This is for<br />
the safety of the student as well as the<br />
teacher. If an adult is not present, the<br />
teacher will conclude the session and the<br />
student will be counted absent.<br />
If the student or someone in the home<br />
contracts a contagious disease,<br />
instruction will not take place. The<br />
parent must contact the homebound<br />
teacher so sessions can be rescheduled<br />
or different arrangements made.<br />
Arrangements should be made to<br />
schedule doctor’s appointments around<br />
homebound sessions whenever possible.<br />
Since a student receives credit for 5 days<br />
of attendance with 4 hours of<br />
homebound services, an absence from a<br />
homebound session could mean that the<br />
student is counted absent for 2 1/2 days.<br />
Student Responsibilities<br />
The student must complete the<br />
assignments left by the homebound<br />
teacher each session. It is suggested that<br />
the student spend three hours each day
on school work. The student should be<br />
organized and ready for instruction each<br />
session.<br />
How Long Does it Take Before the<br />
<strong>Homebound</strong> Teacher Makes the First<br />
Visit?<br />
After the paperwork is completed and<br />
received by all parties, the campus<br />
assessment team meeting will be held.<br />
At that meeting a convenient time to<br />
begin services will be agreed upon by<br />
the parents and teacher. Services will<br />
begin within a week after the campus<br />
assessment team meeting.<br />
Teacher Responsibilities<br />
The classroom teacher and the<br />
homebound teacher work<br />
collaboratively to ensure the student’s<br />
general education program is being<br />
followed.<br />
Classroom Teachers:<br />
Supply assignments, worksheets,<br />
materials and tests to the homebound<br />
teacher. Each school has a<br />
designated area to place homebound<br />
materials.<br />
Evaluate student’s work, assign<br />
grades and return graded<br />
assignments to student’s homebound<br />
teacher weekly.<br />
Assist in making the student’s<br />
transition from homebound to school<br />
a positive one.<br />
<strong>Homebound</strong> Teachers:<br />
Provide instruction in the home or<br />
hospital for the student to gain skills<br />
on weekly assignments.<br />
Administer all exams.<br />
Inform the teachers of student’s<br />
progress and discuss problems.<br />
Return completed work to school on<br />
a weekly basis.<br />
Report attendance to student records.<br />
