Mental <strong>Health</strong> and Substance Abuse Benefits<br />

Inpatient<br />

Expense -<br />

Mental <strong>Health</strong><br />

Covered Medical Expenses for inpatient<br />

treatment of a mental health condition or<br />

for substance abuse are payable as<br />

follows:<br />

Treatment of biologically based mental<br />

health conditions, or rape related mental<br />

or emotional disorders, while confined as<br />

an inpatient in a hospital or facility<br />

licensed for such treatment are payable on<br />

the same basis as any other inpatient<br />

expense.<br />

Treatment of non-biologically based mental<br />

health conditions, or for substance<br />

abuse, while confined as an inpatient in a<br />

hospital or facility licensed for such treatment,<br />

are payable on the same basis as<br />

any other inpatient expense subject to a<br />

maximum of 60 days per Policy Year.<br />

Covered Medical Expenses also include<br />

the charges made for treatment received<br />

during partial hospitalization in a hospital<br />

or treatment facility. Prior review and<br />

approval must be obtained on a case-bycase<br />

basis by contacting Chickering<br />

Claims Administrators Inc. When<br />

approved, benefits will be payable in place<br />

of an inpatient admission, whereby 2 days<br />

of partial hospitalization may be<br />

exchanged for 1 day of full<br />

hospitalization.<br />

Outpatient<br />

Expense -<br />

Mental <strong>Health</strong><br />

Covered Medical Expenses for the care or<br />

treatment of a mental health condition by<br />

a licensed or accredited health service<br />

organization or hospital or by a licensed<br />

practitioner are payable as follows:<br />

Preferred Care: 100% of the Negotiated<br />

Charge after an $8 Copay per visit.<br />

Non-Preferred Care: 100% of the<br />

Reasonable Charge after a $10 Deductible<br />

per visit.<br />

Outpatient treatment is payable up to a<br />

maximum of $1,500 per policy year.<br />
