For questions about ID cards:
A temporary ID card is included with this Brochure. This
card is for identification only. It is not a guarantee of
eligibility or benefits. You will be issued a permanent ID
card as soon as possible. If you need medical attention
before the ID card is received, benefits will be payable
according to the Policy. You do not need an ID card to be
eligible to receive benefits. Once you have received your
permanent ID card, present it to the provider to facilitate
prompt payment of your claims.
Note: Please be advised you will receive a unique Aetna
member ID number on your membership card.
