COMMERCIAL EXHIBITORS<br />
(EXCLUDING EDUCATIONAL EXHIBITORS)<br />
Each exhibiting company is entitled to receive four complimentary<br />
meeting registrations per booth. It is the responsibility of the<br />
exhibiting company to use the complimentary registrations, either by<br />
submitting the EXHIBITOR Pre-registration Form (which will be sent<br />
to the contact person listed on the exhibit application) prior to<br />
May 30, 2008, or by using the complimentary registration on site.<br />
Check with your Exhibit Manager to see if you will be one of the<br />
complimentary registrants for your company BEFORE you register!<br />
If you will not be a complimentary exhibit registrant, please use<br />
the attached Pre-registration Form and pay the appropriate<br />
registration fee. If you have any exhibition questions, please contact<br />

Note: Educational Exhibitors do not receive complimentary<br />
registrations. All booth personnel for Educational exhibits must be<br />
registered for the meeting, paying the appropriate registration fee.<br />
PRE-<strong>REGISTRATION</strong><br />
Pre-registration is strongly advised. If you miss the pre-registration<br />
deadline, you must register on site and pay the higher fee.<br />
Registration forms received after the deadline date will be<br />
returned, and you will be charged the on-site registration fee.<br />
If an e-mail address is supplied, delegates will receive a<br />
confirmation letter via e-mail; otherwise your confirmation will be<br />
sent via fax or mail. You will receive a confirmation of registration<br />
prior to departing for the meeting. If you pre-register and do not<br />
receive a confirmation, please bring a copy of your registration<br />
form and proof of payment to the registration desk on site.<br />
Payment must be received at the time of registration. If no<br />
payment is received with the form, you will be charged the<br />
additional $100 regardless of when the form was received.<br />
Registration forms received via fax or the Internet are accepted<br />
for individuals paying by credit card only (American Express,<br />
MasterCard, or VISA only).<br />
REGISTERING VIA THE INTERNET<br />
You may register for the meeting at<br />
https://www.one-stop-registration.com/iadr.<br />
Within three (3) days of registering, you will receive a final<br />
confirmation of your registration. The information supplied on the<br />
Internet is passed through a secured server, but the <strong>IADR</strong> is not<br />
responsible for any fraudulent credit card transactions. Please note<br />
that credit card payment is required for those registering online.<br />
<strong>REGISTRATION</strong> AND PRESENTER SUBSTITUTIONS<br />
The <strong>IADR</strong> does not allow meeting registration substitutions.<br />
Registration fees may not be transferred to another person.<br />
Anyone unable to attend the meeting after having paid must<br />
cancel his/her registration. The registration fee will be refunded<br />
if the request is received prior to the cancellation deadline date.<br />
The new registrant must then register separately and pay the<br />
appropriate registration fee. If you are a presenter substitution,<br />
the same policy applies. The original presenter must register for<br />
the meeting and then cancel (refund amount will depend on when<br />
the cancellation occurs) before the substitution can be processed.<br />
The new presenter must then register and pay the applicable rate.<br />
ON-SITE <strong>REGISTRATION</strong><br />
Do not complete the pre-registration form if you register on<br />
site (you will be required to complete a different form at the<br />
meeting). Payment in full must be made at the time of registration.<br />
Acceptable forms of payment are cash (U.S. or Canadian dollars<br />
only), traveler’s checks (U.S. or Canadian dollars only), checks (from<br />
U.S. bank payable in U.S. dollars only) and credit card (American<br />
Express, MasterCard or VISA only). The <strong>IADR</strong> will not bill you.<br />
FINANCIAL DISCLAIMER<br />
The <strong>IADR</strong> reserves the right to charge the correct amount on<br />
a credit card if different from the Total Payment listed. Checks<br />
received for payment of goods and services via mail or drop<br />
box will be deemed sufficient authorization to clear checks<br />
electronically through the Automated Clearing House Network.<br />
CANCELLATION POLICY<br />
All registrations are subject to the cancellation policy. Cancellations<br />
must be in writing; telephone cancellations will NOT be accepted.<br />
REFUND POLICY<br />
Requests for refunds for your registration must be received<br />
in writing by the deadline date. Refunds or credits will not be<br />
given for failure to attend, late arrival, unattended events or early<br />
departure. Refunds will be processed after the meeting.<br />

Note: If you need to cancel your Lunch & Learning program<br />
and/or accompanying person registration, a full refund is available<br />
until May 30, 2008. After that date, no refund is available.<br />
