to the Housing Bureau. See pages 9-10 of the registration brochure for all<br />
details regarding room rates, hotel map, reservation changes, and cancellations.<br />
3. Keep a copy of this form. Use one form per room required, and make<br />
additional copies if needed.<br />
4. All reservations are being coordinated by the <strong>IADR</strong> Housing Bureau and NOT<br />
with the hotels directly.<br />
