4. Proposed Funded Service Areas<br />

12. Proposed Funded Service Area Maps: Please attach service area maps <strong>for</strong> each proposed<br />

service area to be funded, identifying <strong>the</strong> service area as: a) unserved; b) underserved; or c) <strong>for</strong><br />

BIP, at least 75% rural. If a proposed funded service area is designated as at least 75 % rural,<br />

identify <strong>the</strong> unserved and underserved rural areas at <strong>the</strong> census designated community level. For<br />

a middle mile project <strong>the</strong> maps must indicate <strong>the</strong> service areas that will benefit from <strong>the</strong> project.<br />

As per <strong>the</strong> NOFA, at least one interconnection point <strong>for</strong> a middle mile project must terminate in a<br />

proposed funded service area that qualifies as unserved or underserved <strong>for</strong> a last mile project.<br />

Purpose of Maps and Requirements<br />

A service area map is required <strong>for</strong> each proposed funded service area. The maps will illustrate<br />

<strong>the</strong> proposed funded service area and identify <strong>the</strong> contiguous census blocks <strong>for</strong> which <strong>the</strong><br />

proposed project will provide service. For middle mile projects, <strong>the</strong> maps should indicate <strong>the</strong><br />

area that will benefit from <strong>the</strong> project and show at least one interconnection point that will<br />

terminate in an area that qualifies as unserved or underserved.<br />

Applicants <strong>for</strong> Last Mile projects should complete Question #13; applicants <strong>for</strong> Middle<br />

Mile projects should skip Question #13 and proceed to Question #14.<br />
