SESSION 3 Wednesday, November 17
Cone Beam CT
Dr. David Gane and Dr. Charles Shuler
Understanding Cone Beam Computed Tomography (CBCT)
Three dimensional CBCT technologies provide the practitioner with an accurate
representation of the hard tissues of the teeth, face and jaws and also the
airway. It is no wonder that this amazing technology is transforming the way
that dental professionals diagnose, treatment plan, and execute treatment.
This presentation will discuss the very latest advances in 3-D radiographic
imaging technologies and demonstrate the diverse range of clinical
applications using clinical case studies.
SESSION OBJECTIVES
• Learn the technology & fundamentals of CBCT
• Present the basics of 3D visualization & overview of 3D anatomy
• Review the clinical applications of CBCT
• Understand CBCT dosimetry
• Create a better understanding of CBCT imaging
Radiographic Oral Pathology: An Anatomic Approach to Differential
Diagnosis
Traditionally oral pathology has been taught in dental schools by presenting
either clinical, radiographic or histologic images each with a specific diagnosis
attached and subsequently asking the student to recall these names. In
clinical practice alterations in the hard tissues rarely resemble the images
from either dental school classes or oral pathology textbooks. The objective of
this presentation is to guide the participants to develop an approach for the
differential diagnosis of lesions of the hard tissues of the oral cavity through
evaluation of key features of the lesions and reference to the anatomy/
histology of the intraoral site. This approach builds on dentist’s strengths
resulting from their clinical care of patients to provide a more comprehensive
strategy to develop a differential diagnosis.
The importance of developing a differential diagnosis will be stressed in this
presentation in order to insure that important conditions are not overlooked
when evaluating a patient. In contrast to the expectations in a dental school
oral pathology course it is not important to make the diagnosis of any oral
lesion clinically the first time it is observed but rather to consider the range
of possible diagnoses and establish a priority for them. Procedures that lead
to a definitive diagnosis will be discussed and the rationale for selecting a
particular procedure presented. It is important that getting the diagnosis
Continuing Dental Education • Fall calendar 2010

11
“right or wrong” at the chairside or in the clinic should not be the goal. The
goal should be consider all the possible diagnoses and proceed along a
course that is most beneficial for your patient.
The presentation approach will use specific clinical cases to demonstrate the
process for generating the differential diagnosis and determining the proper
course of action to reach a final diagnosis. The steps involved in generating a
thorough assessment of the lesion will be presented so that a broader range
of potential conditions is considered. The result of this clinic will be a new
strategy to approach the assessment of radiographic oral lesions that will
result in a more extensive set of differential diagnoses.
LEARNING OBJECTIVES
1. To recognize changes in oral hard tissues representative of pathologic
changes
2. To evaluate the radiographic lesions in a standardized format based on
using clinical examination strengths generated in practice.
3. To develop a process for generating a differential diagnosis based on the
anatomy and histology of the site
4. To understand the application of additional procedures to reach a
definitive diagnosis and determine an approach to treatment
5. To apply the process to clinical cases to reinforce this new approach to
the evaluation of oral hard tissue lesions
David Gane, DDS, is a graduate of the University of Western Ontario with
an honours degree in physiology & pharmacology and a doctorate degree in
dental surgery. Dr. Gane has authored many articles and technique videos on
digital radiography and has lectured nationally and internationally on digital
imaging topics. He is the clinical director for Orbit Imaging Centers and serves
as Vice President of Dental Imaging for PracticeWorks Inc. the exclusive maker
of Kodak Dental Systems.
Charles F. Shuler, DMD, PhD, is the Dean of the Faculty of Dentistry of
the University of British Columbia. Prior to UBC he was a faculty member
at the University of Southern California for 18 years. At USC he served as
the Director of the University of Southern California Center for Craniofacial
Molecular Biology holding an endowed chair position as the George and Mary
Lou Boone Professor of Craniofacial Molecular Biology. He also served as the
Director of the Graduate Program in Craniofacial Biology and the Associate
Dean for Student and Academic Affairs at the USC School of Dentistry. Dr.
Shuler received his B.S. in Biochemistry from the University of Wisconsin,
his D.M.D. from Harvard School of Dental Medicine, his Ph.D. in Pathology
from the University of Chicago and his Oral Pathology specialty education
at the University of Minnesota and the Royal Dental College Copenhagen
Denmark. His current research interests include craniofacial development,
oral carcinogenesis and gene therapy.
SESSION 4 Wednesday, December 8
Behavioural Management for Dental Treatment from
Paediatrics to Geriatrics
Dr. Martin Aidelbaum and Dr. Phoebe Tsang
Dr. Aidelbaum’s lecture will cover behaviour management for the aging and
geriatric population. Topics will also include management the alcoholic patient
and patients with Alzheimer’s disease, and related dementias.
“No, but I don’t want to…”: Your 4 year-old patient’s protesting voice pierced
through your ears when you were trying to anaesthetize her for a restoration.
Sound familiar? The restoration is the easiest part of the dental appointment
when it comes to dealing with little people who present with different
personality attributes. The objective of this seminar is to review patient
selection and behaviour guidance techniques which you may find helpful in
calming your anxious paediatric patients and lowering your heart rate and
blood pressure during a dental appointment
16 Cast Gold Restorations – Some New Thoughts
With the addition of a few simple techniques, the perceived difficulties of cast
gold restorations can be eliminated; and the enjoyment of providing exceptional
dental treatment for your patients can be realized. No dental material can match
the aesthetics of perfect enamel, especially after a few years have passed.
However, the use of a combination of cast gold restorations, which provide
longevity second to none, and the careful preservation of the aesthetic tooth,
can yield the perfect restorative solution for many of our patient’s needs. The
key steps involved to enable dentists to provide a more exquisite treatment for
their patients will be presented.
Ruddle on Rotary
This presentation will demonstrate a technique for shaping canals and cleaning
root canal systems in preparation for three-dimensional obturation. Dr. Ruddle
will emphasize the essential elements for success, including access, the irrigants,
and the strategic role of small-sized stainless steel hand instruments to create
or verify the glide path. This presentation will focus on the unique features of
the ProTaper rotary and manual file systems, their method of use and how to
consistently create fully tapered shapes. New, innovative methods for enhancing
debridement, smear layer management, and biofilm disruption will be shown.
At the end of the presentation, participants should be able to:
• Identify the biological and clinical factors that influence success.
• Utilize “microsonic” techniques to refine and finish the access preparation.
• Understand how the design of a file will influence performance.
• Create a predictable glide path for rotary files to follow.
• Shape canals to facilitate 3-D cleaning and filling.
Treating Patients With Excess Gingival Display:
A Collaborative Approach in Creating Age-Appropriate
Smile Aesthetics
In today’s world of “extreme makeovers”, more and more patients are seeking
dental treatment to soften the effects of aging by creating a more youthful
smile. In their quest to achieve certain idealistic aesthetic objectives, should we
consider it reasonable to create the “30-something” smile for every patient?
Or should we be setting more realistic objectives to achieve aesthetic goals
that are more age appropriate for our patients? When planning orthodontic
treatment for patients with excess gingival display or “gummy smiles”, it is often
necessary to also incorporate surgery and/or restorative dentistry to achieve
ideal smile aesthetics. Dr. Kokich will identify the threshold levels at which these
gingival discrepancies become noticeable as well as the various diagnoses
associated with excess gingival display. He will also discuss the parameters
necessary to create age appropriate treatment alternatives for these patients
while emphasizing the importance of utilizing an interdisciplinary approach.
Dr. Alan Lowe
Oral appliances are indicated for use in patients with primary snoring or mild
to moderate obstructive sleep apnea and in patients who are intolerant of,
or refuse, nasal continuous positive airway pressure. At present, more
that 100 appliances are available on the market but only six have been
adequately evaluated in randomized clinical trials. Recent research findings
have significantly increased our knowledge of when oral appliances should
be used and which appliances are best suited for specific patients. Results
from recent randomized control clinical trials that compare the subjective and
objective efficacy of specific adjustable oral appliances and nasal continuous
positive airway pressure will be discussed. The indications, contraindications,
advantages, disadvantages and side effects of the currently available oral
appliances used in the field will be described in detail. Particular emphasis will
be placed on the interaction with the attending physician or sleep specialist and
the detailed dental clinical protocols and procedures involved.
Combining Science and Technology for Endodontic Success
Dr. Fred Barnett
PROGRAM DESCRIPTION: ½ day lecture & ½ day workshop
Endodontic diagnostics, instruments and techniques have undergone
revolutionary changes in the last several years. If the endodontic techniques
you currently use in practice were acquired more than 5 years ago, that
technique is now outmoded and obsolete. It is now well recognized that Nickel
Titanium rotary files, if used correctly, are safe and offer a better outcome for
treatment. In addition, new thermoplastic resin root canal filling materials are
easy to use; offer improved sealing properties and will replace the routine use
of gutta-percha.
This lecture and hands-on course will provide an opportunity to update and
familiarize you with these contemporary advances in Endodontics. Upon
completing the course, you will be prepared to perform endodontic procedures
in a more straightforward and predictably safe and successful manner.
