Please type in 12 font using New Times Roman and single spaced the answers to the
following questions. You must write the question number beside each of your responses.
1. Write your name, email address, partner’s name, and partner’s email address if
applicable just like the top of this piece of paper.
2. Indicate start date as well as the expected completion date.
3. What is the name of your organization/school that you are working with? What is their
contact information?
4. Write 2 paragraphs (4-6 sentences each) describing your project. Make sure you
address the following: What needs are you addressing in the community?
5. Where will your project take place?
6. Materials and Budget: List all the materials needed and projected costs that you will
incur.
7. What are your challenges and limitations in your project? How will you handle each
challenge and limitation?
8. How are you demonstrating leadership? (Be specific and detailed)
9. Do you have adult supervision, if so who?
10. Describe expected costs and how you will cover them. If you plan on fundraising,
indicate how.
11. If you are working with other BMHS or non-BMHS students, you will need to use Sr.
Service Project Permission Slip (see Campus Ministry). Write “Yes” for question 11 on
your typed answer sheet if you will utilize other students.
12. Write specifically the steps that you need to take in order to complete your project.
Beside each step you need to write the date you will complete the step by.
