Associate in Applied Science Degree in <strong>Nursing</strong> Procedure for Entering into the <strong>Nursing</strong> Program 1. Submit a completed New Student Information <strong>Form</strong> to Admissions. 2. Meet with a counselor to establish a schedule for taking prerequisite courses. Call (630) 466-7900, ext. 2361. 3. Obtain specific admission information by contacting the Healthcare Programs office at (630) 466-7900, ext. 2322. 4. Take the required Pre-Admissions Program Assessment Test (NOTE: For first consideration, test scores should be recorded by the application deadline. However, scores received by January 1 MAY be considered for Spring and scores received by August 1 MAY be considered for Fall if space is still available in the program.) Before July 1, 2014: Take the required Pre-Admission Exam – RN (PAX-RN) assessment test. a. Contact the Center for Learning Assessment at (630) 466-7900, ext. 5700 for <strong>Nursing</strong> Program Assessment testing details such as fees, availability, and scheduling procedures. b. Acceptance into the program is based on assessment results, with documentation of verbal, math, and science of 55%, as well as a composite of 65% for the PAX-RN. A student has two opportunities to successfully meet assessment requirements. If a student fails to successfully meet assessment requirements on their first attempt, eight weeks must elapse between testing sessions for the PAX-RN assessments. Due to the eight week retest policy for each assessment, students should plan accordingly for the initial test and possible re-test. Guidance is provided by the Learning Enhancement Center and the Center for Learning Assessment for students who do not meet these entry requirements. Test scores are only valid for 24 months. Students who failed the PAX-RN two times prior to July 1, 2014 will be given one (1) opportunity to attempt to meet the TEAS requirement. The opportunity expires July 1, 2015. July 1, 2014 and after: Take the required TEAS (Test of Essential Academic Skills) assessment test. a. Acceptance into the program is based on assessment results, with documentation of Reading, Mathematics, Science, and English and Language Usage of 55%, as well as a composite of 55% for the TEAS.
