For first consideration, test scores should be recorded by the application deadl<strong>in</strong>e. However, scoresreceived by January 1 MAY be considered for Spr<strong>in</strong>g semester and scores received by August 1 MAY beconsidered for Fall semester if space is still available <strong>in</strong> the program.How do I schedule an appo<strong>in</strong>tment for program admission test<strong>in</strong>g? You need to schedule your appo<strong>in</strong>tment <strong>in</strong> person at the Center for Learn<strong>in</strong>g Assessment, Sugar Grovecampus. Individual appo<strong>in</strong>tments are available throughout the year, Monday through Saturday, exceptdur<strong>in</strong>g the month of December. The tests are adm<strong>in</strong>istered only at the Sugar Grove campus. A non-refundable fee of $50.00 is due at the time of <strong>in</strong>itial test registration. You may reschedule yourappo<strong>in</strong>tment only one time per each $50.00 fee. Allow a m<strong>in</strong>imum of four hours to complete the TEAS V test. The day of the test, only a government issued photo ID with a signature and a permanent address,such as a driver’s license or a passport will be required, X number cards will not be accepted. TEAS V Retest InformationAs of 8-01-2006, students can only test twice <strong>in</strong> a lifetime for the Nurs<strong>in</strong>g program. All prior NET testscores and PAX test scores are <strong>in</strong>cluded <strong>in</strong> this count.For example:a) If you tested <strong>in</strong> 2009 with the NET, that counts as test number one.ORb) If you tested <strong>in</strong> 2010 with the PAX and your scores expired, that counts as test number one.Exception: If failed the PAX twice, you have one chance to take the TEAS test. This opportunity totest expires on 7-01-15.Must wait eight weeks from your <strong>in</strong>itial test to take a retest.
