• Ensure patient knows what type of appointments need to be made
(cardiologist, physical therapy, wound center, etc.) and the phone
numbers.
• Review timeframe that appointments need to be scheduled.
Patient/Family will:
• Write information on SMART worksheet.
• Ask provider for office phone numbers they do not know.
